My apologies in advance for digressing on the Indonesia thread, but the replaying of 9/11 scenes today bring back unpleasant memories. I was within a block and a half of the Oklahoma City bombing when it happened back in 1995. Horrid things really do happen in the midst of everyday, normal life.

My thanks to all who are working to keep H5N1 from becoming a tragedy.

Michelle, so 4, maybe 11, new cases in last 2 days. I had a sense it was intensifying, as well as the concern over the Akhira cluster from June. I just keep reading article after article about thousands of chickens dropping dead all over Sumatra. “death incident of several cattle livestock after preying on the chicken died this”

Hogs are the only type of “cattle livestock” that will prey upon sick or dead chickens. If they died afterward, and the people are starting to get sick,…well,…this ain’t good. Sounds like folks are heading for the hills (Village Rumoong Upper).

FrenchieGirlat 19:06

The term cattle usually denotes cows in the US, but cows “never” eat chickens. It is being used in a more broad sense here. Hogs will eat almost anything. They are quite omniverous. Give them a chicken and they will scarf it up and ask for more. If this news is valid, it is very bad news. Sorry about the nightmares. Better in your dreams than in your waking hours.

In regard to Influentia2 at 17:22. It sounds as if the laboratory in Jakarta is biosecurity level 2, and the one in Hong Kong is level 3 (probably as high a level as the CDC, etc.) It sounds like Jakarta is trying to come up to the standards of a level 3 facility. It sounds like they would like to do all their own testing on Indo soil, research etc. But, if all they wear are N95 masks, and some gowns and gloves, then it will never come up to the highest level (scientists working in space suits behind thick biosecurity shielding thru astronaut gloves and only breathing thru ducts that supply air from an outside source—you get the picture!). I thought that sentence was very telling.

Cygnet – at 23:09

re: dogs being paralyzed after eating dead chickens.

For what it’s worth, chickens have pretty disgusting eating habits. There’s not much they won’t eat, including things generally considered inedible, like manure and VERY dead animals. While it’s difficult to give a chicken food poisoning, in hot and wet weather they can get BOTULISM from scavenging dead animal matter. Botulin toxin is wicked nasty stuff and it takes very little to cause paralysis — I can easily see dogs being paralyzed by eating chickens that died of botulism.

Symptoms of botulism? Drooling, incoordination, limp paralysis … starting in the lower extremities and working up. Seems to fit, anyway.

Rabies does NOT fit. It has a long incubation period and rabid animals generally do not eat or drink because of spasms of the esophogeal muscles.

Cygnet – at 00:22

pugmom — it generally kills the chicken. I will assume from what I know about CANNING and botulism that after killing the chicken the clostridium bacteria will continue to grow and produce toxin, so the older the carcass, the more toxic it would be. Note this is my assumption — I don’t know for sure.

(Symptoms in chickens are incoordination, a limp neck, and death, iirc.)

This is pure speculation, but it would explain why the dog(s) got paralysis after consuming dead birds.

MaMaat 00:38

From the report on the dog suffering paralysis after eating the chicken… I was confused the illness what attacked chickens in the Rumoong Village, his article the dog that in the healthy condition after eating the chicken carcass that was allowed just like that immediately was slavishly paralysedNontje also explained although already in the paralysis situation, his property dog was still continuing to eat

Wouldn’t ingesting botulism, enough to cause paralysis in the dog, also kill the dog though? It sounds like the dog was continuing to eat, though salivating heavily, after the paralysis set in.

There is another article on that website that reports the test results for a number of poultry in Rumoong Low as being postive for bird flu.

Michelle in OK – at 08:07

I think this is the same patient pugmom posted about at 20:35. The initials are different, but the hospital and date of admission are the same. We now know an age, occupation, and that he played with the chicken.

Okieman – at 08:17

Folks, concerning the dog that was reported as “paralysed”, I might point out the influenza has been known to cause neurologic symptoms in humans and this might just be another manifestation of what it can do to dogs. There is no way to know and I wouldn’t place to much emphasis on how one dog got sick. In addition, Toggletext as we know, is not all that accurate at times. Everything points to H5N1 causing the chickens to die, the dogs to die, and now humans to get sick. The test results support this.

I am afraid we are about to see several clusters in multiple locations in Sulawesi. We also need to start watching Borneo closer because it is next in my opinion. I’m already watching New Guinea, it is mostly a news black hole, at least in the interior of the island.

pugmom – at 12:42

I follow the news closely in Indonesia (sometimes, I think, too closely). I am scared. Sometimes I feel like I am going to blow a gasket, the news is so dire. The situation is ominous. They are sending in military troops to force culls, they are setting up make-shift BF hospitals to deal with all the potential “casualties,” the Indos are worried about the rains coming any day, there are more and more human suspects popping up everywhere in all locations, the authorities are appealing for calm and no panic, the villagers are heading for the hills, there are scads of unresolved diagnoses and cluster cases, the WHO is silent, Tamiflu is being handed out like candy, the chickens are dropping like flies, the Indos are even talking about having to import chicken meat, need I go on. We have little commentary on this thread, which is good, as we have other threads in which to discuss PPF, etc., and we need to keep an open channel for just case reporting as it is going fast and furious. But, sometimes I wonder if our fellow Fluwikians are reading the articles on this thread. Or are you all just sitting back in silent horror and watching this unfold in Indonesia, as I am. Are we going to sit back and wait for the WHO to announce level 5 (which I believe is almost there in Indo) before anyone gets excited? I know, I know, I need to calm down and go back to just researching articles, but by gosh and by golly, my own personal feeling of impending doom is growing ever heavier.

Snowhound1 – at 14:15

pugmom…thanks for the little “rant”. I am copying your post and sending it to some people, who are prepping, but I don’t think are fully aware of where we may be at the moment. Hope you don’t mind. I think it is a pretty good summary of what appears to be a pretty bad situation in Indonesia. I also posted a few minutes ago on the news thread how Vietnam will begin checking temps on all incoming passengers…anyone with a temp over 38C, will be whisked off to “isolation”. Makes me really wonder if they know something that we don’t.

pugmom – at 14:22

Snowhound—good luck with trying to win any converts. I showed the spreadsheet summary to my best girlfriend yesterday and her eyes waxed over and she couldn’t conceal her yawns.

Desert Dan – at 14:23

I can’t keep track of the lastest in Indonesia? Hard to follow the names and locations etc.

What is the running total of deaths (documented or suspected)from AI since the July-Aug outbreak?

What is the total number of suspected AI cases(still living)from the same period?

How many clusters? or are the suspected sick people all over Indonesia?

pugmom – at 14:34

Hi Desert Dan—the best way to try to get a handle on it is to go to the wiki to view Michelle’s spread sheet (password wiki123) and then to copy it out. Since she did that spreadsheet 2 days ago, the suspected cases have climbed by another 8, maybe 11, cases.

Ree – at 14:42

DD 14:23 - This is what is so tough about this thread. Pugmom and others have been working non-stop to translate and pull these reports out of places I don’t even know about. I find it’s hard to take the “time” to keep track of it all - you might miss something really big happening if you do.

Pugmom - You and your crew are so incredibly unselfish with your time. Thank you, thank you for what you do. Yes, we are here reading very carefully. And prepping harder because of it.

Right now I think we have a window that’s frosted over. Indonesia is a little hole we’re breathed on really hard and are looking through it. We can’t see very clearly. We can’t see what’s going on around the edges.

But we can’t stop trying to see as much as we can through our little hole.

pugmom – at 15:11

As many as 21 cases of bird flu were in this province found with the number of death rates 16 people….

Yudi said, in West Java the positive case of bird flu to humankind was found in 14 city regencies among them in Bekasi, Bogor, Garut, Sumedang, Indramayu and the Bandung Regency…..

In West Java was prepared seven reconciliation hospitals to handle the sufferer suspect bird flu among them in the Hospital (RS) Handsome Sadikin Bandung, RS Dr Slamet Garut as well as RS in Indramayu and Cirebon….

Blue Ridge Mountain Mom – at 15:46

Ok, stupid question time. For some reason, the pugmom - 15:35 news story has frozen my brain.

When it says 14 city regencies does that mean 14 cities or 14 counties or 14 provinces? I was just trying to compare to the map and only Garut is on our map which would really point to some glaring holes in our news. Of course the 7 hospitals set up to handle suspected bird flu patients is telling us a lot in the ‘watch what they do’ category.

INFOMASS – at 16:18

It is possible that the avian flu is spreading among chickens and the increased # of infected chickens is also producing an increased # of infected humans. This does not mean that the flu is not also becoming more easily spread from chicken to human. If it had reached a “real” H2H pandemic level, we would probably be seeing thousands of cases a week. The “Tamiflu blankets” they are throwing around does complicate any attempt to understand what is really going on, and the situation is certainly very serious though (in my view) short of the level 6 we all fear. If the numbers of villages with infected chickens keeps going up, and they keep trying to hand out Tamiflu to everyone, we should observe shortages of the medicine in a bit. I doubt there is enough Tamiflu for even 5% of the Javanese population (=6 million) and at that point the “real” situation might emerge. Let me thank pugmom and others for incredible work. They could be the sentinels in this whole awful business.

Emergency cull

Local officials in Indonesia’s West Java province have ordered an emergency cull of some 4,500 chickens to contain a new outbreak of the bird flu virus among the poultry population, a local official said Tuesday. No human infections were reported from the quick spread of the H5N1 avian flu virus in two villages of West Java’s Kuningan district last week, which killed around 1,500 chickens. Asep, an official from the Kuningan’s district government office, said workers immediately began culling poultry in those villages and in surrounding areas last Saturday to contain the outbreak. “We will continue until (Sunday). After the outbreak was positive, we decided to cull.”

Michelle in OK – at 16:46

Blue Ridge Mountain Mom at 15:46… there are no stupid questions, and I am certainly not the expert when it comes to geography, but I’ll try to address your concern.

One major problem we run into trying to sort out patients is the different levels of information in various news reports. Sometimes they list village names, sometimes a street location, sometimes only the hospital location such as Bandung, Garut, etc. We try to capture the level of detail available, so that we are less likely to confuse patients in the future.

If you look at the spreadsheet at the link provided above by pugmom, you’ll see several patients in the Bandung hospital, but none of them are shown as Bandung on the map, because we had more detailed information (such as a subdistrict or village) available for them.

Regarding the two patients shown as Garut… we don’t have additional information for them, so that’s as close as we could get. The attached link contains an article with descriptions about cities/subdistricts, etc. I hope that helps. Another Thread

As usual, please do bring up inconsistencies you notice so that our data will be as accurate as possible (considering the huge gaps we’ll probably never know.)

MaMaat 16:48

Blue Ridge Mountain Mom at 15:46, from wikipedia…. “A regency (Indonesian: kabupaten) is a political subdivision of a province in Indonesia. The Indonesian term kabupaten is also sometimes translated as ‘district’ or ‘municipality’. Regencies are divided into sub-districts.”

Sometimes a regency is a city, sometimes it’s more like a county. Hope that helps.

Okieman – at 17:17

I think I have this right and am not muddying the water:

Province = U.S. State, example- West Java

Regency = U.S. County (or Parish if in Louisiana), also can be a large city

Sub-regency or sub-district = U.S. Township, also can be a large city

Village = village

In making the map I use what Michelle has on her spreadsheet. As she mentions in her post above, if there is not village level info, then…well…we use what we got. Unfortunately, I have been unable to locate a GIS data set that breaks the country out into Regencies. I have “name” data for the Regencies, sub-districts, and villages, but I am unable to show the boundaries of those political entities. As it is, I’ll just continue using what is available to get’r done (in true Okie fashion). If you start to see pieces of duct tape on the map showing where the Regencies are located you will know that I have finally hit bottom in the GIS mapping department;-)

EXCERPT: Kandun said two other patients had fallen ill with bird flu, a 27-year-old woman from West Sumatra - whose sister earlier tested positive for H5N1 - and a 14-year-old girl from South Sulawesi.

Both were receiving the anti-viral drug Tamiflu and under close observation, he said.

I finally see some of this being reported elsewhere for a change.

Michelle in OK – at 08:12

Influentia2, I’m a little baffled by the above post. No one has been reported as testing positive from West Sumatra in all of 2006. And now her sister is ill? If the sister goes back to 2005, that seems a little odd. (Perhaps the reporter should have said W. Java… maybe more info will surface.)

I hope more surfaces on the 14 year old girl as well. If this is a new case, hopefully she is not related to the 14 year old Akhira who died (and who had relatives who died untested.)

Michelle in OK – at 08:31

AnnieB, thanks. That is a good idea so the people who want to check quickly can get information all in one location.

It took a little pondering to get the spreadsheet formulas set up to calculate automatically without extra time spent sorting, etc. to develop summary numbers. Now that it is set up, it shouldn’t be a problem to copy it in each night.

Where does everyone want the information posted? I’ll put it in the sandbox, on the Indonesia Summary thread, and here as well, unless someone speaks up with a different preference.

Hi, guys, I was watching the ticker on Fox News last night when they said that the 49th case had been confirmed in Indonesia. It was from March and was a 5 yr old boy. The next little blurb said that he was positive now because WHO had relaxed their testing standards. Anyway, this little guy is before our time frame, but I think that he is still very important. I’m looking for the official news story on-line since it was pretty late last night.

Indonesia Raises Bird Flu Death Toll to 49 By VOA News 13 September 2006

Indonesian Health Ministry officials say a five-year-old boy who died in March has been declared the country’s 49th bird flu death.

Officials said Wednesday the victim was added to Indonesia’s list of bird flu casualties months after his death because of a change in the World Health Organization’s testing guidelines.

Health officials say the U.S. Centers for Disease Control and Prevention, in the U.S. state of Georgia, confirmed the boy died of the H5N1 virus in March in a Jakarta hospital….

I wasn’t aware that he was dead; just that they had confirmed him positive. I know that he’s out of our timeline, but I thought that he was important. I’m going to go cry in my coffee now. Not another little one….

Hi Michelle and all. I agree that posting the tallies in the concise way you have done will be so helpful to all the Fluwikians that find this thread difficult to follow. Wonderful idea too AnnieB—thanx so much. It is hard not to see what is unfolding when one has the raw numbers in front of them. Michelle, I spent alot of time night before last trying to round up info on the cases where results have disappeared into thin air, or we haven’t heard outcomes. I, for the most part, was unable to find out any new information, even though I googled way late into the night. I guess we may never know what happened to alot of them. I believe, however, that Indo has a free enough press, that if they had died, we prob would have heard about them. Our suspect numbers may remain high and unresolved.

Question regarding the article posted by Influentia2 at 4:57 - Should I show these two individuals as suspected cases or positive cases? The article says they “fell ill with bird flu.” This indicates to me a positive test, but I hesitate to post positives without additional information. What do you all think?

Frenchie Girl 15:22

I have seen UNICEF in several articles, including East Java. I also have one somewhere telling about Save The Children helping in North Sumatra with disinfecting chickens. I have read about these before I posted on FW and might be able to find them for you if you’d like. I posted one last week about “the non-governmental agency” helping with disinfecting and spreading the word about BF in East Java. Everything that was quoted as coming out of the loudspeaker on the truck translated into perfect English, I thought that was very unusual, but this organization was only referred to as “non-governmental”.

FrenchieGirlat 17:00

If UNICEF has a regular post in that area of Indonesia, then I am calm. But if they came, ‘as a delegation’ to visit what was going on, then I’ll get nervous. UNICEF is United Nations, and, by extension, the eyes of WHO. Hence my question. I’ll try to find out tomorrow through my own channels, but it definitely would be easier if someone had an answer on the tip of their fingers. Roaming through the UN internal web site is a huge task and I happen to have to work for a living, too! <smile>

That was part of the article about the 5 year old boy WHO died in March. I am curious if more people around him were sick or died like in the case of Ahkira.

As far as the 27 year old and 14 year old I don’t know, but maybe we missed them or didn’t count them for some reason. Or maybe they weren’t reported, I just don’t know.

FrenchieGirlat 17:03

Influentia2 — at 16:55 - your post crossed mine. I’ll do as said, that is assuming 1. I find this information, 2. It is public and I can use it. Now is almost bed time for me… It’s not a question of UNICEF delivering aid in trucks for example, it’s knowing if they actually have a building/office in that area. If not, I’ll start being worried.

Muhammad Farhan: From talk shows to bird flu campaign

Kurniawan Hari, The Jakarta Post, Jakarta

When officials from the United Nations Children’s Fund (Unicef) asked him to help with their campaign for the prevention of bird flu a few weeks ago, talk-show host Muhammad Farhan, 36, accepted immediately.

He only demanded one thing — time. Time to sit together, to share views and to prepare for the project.

“We (the Unicef people and I) spent time in discussion. We did not debate the (campaign) schedule, but rather discussed the targets, the platform and other aspects of the project,” Farhan told The Jakarta Post recently.

The results of these preparations are obvious.

During a one-hour interview, Farhan clearly explained how the public at large can help prevent the spread of bird flu. He did not use jargon or technical terms but straightforward language understandable to ordinary people.

“Bird flu relates to everybody’s lifestyle. If we lead healthy lifestyles, we won’t get infected.”

“There are two important things. First, we should clean our hands before having meals, drinking or smoking. Second, if we cook chicken or other poultry, we should cook them well. Do not eat half-cooked eggs,” he said, referring to the preference of many Indonesians for eggs prepared in this way.

For those who breed chickens or raise singing birds, Farhan suggested they should keep them separate from their own living space. People, he said, sometimes bring chickens into the house when it rains.

“Don’t do that because it will increase the risk of bird flu. People should clean their bird cages regularly,” he added.

Farhan acknowledged that his decision to get involved in the bird flu campaign has been driven by personal cicumstances. He lives in Tangerang, Banten, an area where there is a high prevalence of bird flu.

Nine out of 14 suspected bird flu patients in Banten have died, according latest data.

“Besides, I have two kids. I want the public at large to share my concerns,” he said, adding that, coincidentally, Unicef is promoting the concept that bird flu can be prevented…

I do not think this is the same Gilang. I think this is the pre-schooler listed eariler in an article dated 9/12 (this thread).

This article lists the same hospital to which the child was transferred.

Banda Lampung (or Bandar Lampung) is the capitol of Lampung in the region of sumatra.

Those are my thoughts anyway…
