FloridaGirl and Michelle—trying to pinpoint where these people are from: confusing, as Edi and Al are both 42 years old. But Edi is from the south and Al is from the north. About the preschooler from Medan: I went back to Michelle’s post at 22:26 and retranslated the article and indeed it said she was treated from 9–9−06 to 9–12–06, at which time she tested negative and was released to home with her family. The street she lives on is Jamin Ginting.

I have lost the directions of how to print out the excellent map in the case summary thread. Someone told me about a week ago and now I can’t find it. I was thinking, whoever knows, maybe you should post the directions on how to do it in that thread itself, instead of here, where it gets lost. I was hesitant to print it at first, as it takes alot of ink, but I have surfed the net for a map as clear and detailed and cannot find one. Do you have a link where you got this map? Thankyou very much.
pugmom, the map on the summary thread is a .jpg file, so you can save it to a picture program and size it as desired to print. As Okieman pointed out on an earlier thread, an .html file is a lot clearer, but there is not a link to that type of file now. The one I saved to my favorites won’t display anymore.
Michelle-thanx! What map do you use? Do you have a favorite one that you have printed out? Also, wanted to thank you for putting the fact they died under the “Other info” column. By the time I get all the pages scotchtaped together, the far columns on the right don’t always line up. Great summary, very easy to access. I like the new cases in Blue—that really helps alot to.

I’ve noted the new retrospectively confirmed cases announced by the WHO as discussed on the Sept. 14 News Thread. They are outside of our time frame, but I’ve shown them on the sheet with prior cases. I think the (27M) developed symptoms 10 days after his (15F)sister… both recovered. She was discussed on the WHO update #15.

Influentia2--yes, I did see that, I remember reading the part about burning the chicken coop down to ash (they have a way with words.) It seems like every article I pull up has new regions/villages, etc coming down with new diagnoses of avian influenza. It is hard to keep up with peoples’ names, much less all those foreign village names. All I know is it is spreading like wild-fire. (I find it strange Aceh, way up in the corner, has not had any reports yet.)

FWIW, Newcastle is endemic in Indonesia and it’s got a cfr of close to 100% in domestic poultry. Wicked nasty virus. (Also, in the same virus family as influenza, if I remember right, though not much of a zoonosis thankfully.)
OTOH, symptoms of newcastle and symptoms of AI are alike. I do hope they’re actually testing those birds.
Almost all of the articles, except one, has declared an epidemic of AI AFTER all the testing procedures have been done. What have you found Influentia2??

The Fixed column on the left has the location, name, relationships, and a second line (below) contains all the information about the patient, when they had symptoms, what day admitted or transferred to the hospital (and name), contact (or no) with chickens, and so on. In order to read that easily, all you have to do is click on the box under the name and read it.
The entire right section is moveable, and has dates accross the top. there is a key at the top of each page that tells you what colors represent what. i.e. yellow bars cover the dates symptoms were present, blue bars cover the dates the patient was in the hospital, red covers the day the patient died.
The purpose of laying this out in this fashion is to show the relationships of time elasped between onset of symptoms of people in the same locations who are NOT family members.
If you have not seen the genogram which clearly show the relationships of clusters, you should also look at that.

Here is something to think about. I was surfing the other forums and one of them is commenting on how there have been no reported deaths in the media since Aug 17th or 18th. That is a little under a month. And yet, many reported suspect cases, with no follow-up. Do you think there is anything weird about this? Take, for example Ahmad Djafan the Dawn, a 1 1/2 y.o. from Kendari that was put into ICU on 8–25–06. Then nothing more. Or what about the 7 suspected cases being treated “intensively” since 9–9−06 in Tolitoli, Central Sulawesi? Did they all make it? Five days is a long time for someone in critical condition. We should have heard something, anything. Sometimes I wonder if we are spending all this time tracking down people with just bad colds. Maybe there really haven’t been any deaths in the last month, but then again….

Sept. 14 (Bloomberg) — A 27-year-old Indonesian man with bird flu may have caught it after spending six days caring for a sister hospitalized with the disease, the World Health Organization said.
The case brings the country’s infections to 65 people, including 49 deaths, the United Nations health agency said on its Web site. The man, from Solok in West Sumatra, developed mild symptoms in May and recovered in a few days.
An investigation determined he had exposure to his sister during her hospital stay, and that human-to-human transmission could not be ruled out as the source of his infection, the agency said in a statement today. He reported no contact with dead or diseased poultry before falling ill.
The case represents another instance of possible human-to- human transmission and raises questions about the strain of H5N1 spreading in Indonesia, said Ira Longini, a University of Washington epidemiologist who advises the U.S. government on flu issues. Health officials are concerned that the H5N1 bird flu, which has killed more than half of those infected, may mutate into a form that spreads easily among people.
It’s still an avian strain, but it could be more adapted to human populations, Longini said today in a telephone interview. These human clusters of cases in Indonesia with apparent human-to-human transmission are great cause for concern.
Initial Tests
The man’s initial tests for H5N1 were negative, the WHO said. Follow-up testing of his blood showed a significant increase in antibodies to the H5N1 virus, which met the agency’s criteria to confirm the infection.

Influentia2… could be, but the earler article specifically states that the 27 year old from West Sumatra was a female. It also alluded to the fact that the 2 cases were in the hospital and taking tamiflu. I’m hesitant to take off the (27F), but am hopeful that she isn’t a separate person and somehow related.
Pugmom, I am concerned that there is some tension between authorities behind the lack of information. Cikelet has concerned me for some time, as there was obviously a cluster which occurred. There are (or were) 18 people ill in Cikelet for whom test results have not been released. One of these people is 5 year-old Risman, a sibling of Rubiah and Misbah who both died without tests performed. The last we knew, he was hospitalized with symptoms.
This concern is what prompted the addition of the third sheet in the Upload file dated 9–09 which is called “Sorted for Unknown Test Results.” It highlights the holes in the information. If you haven’t looked at it yet, to me it is troubling, as well.
If these ‘retrospectively confirmed cases’ represent some sort of concession to work out any tension that exists, I hope they get on with it.

This is really confusing. Manado is a city, and I saw a reference to Wonasa as a district in Manado, but Ternate is a city on a small island to the east (across the Molucca Sea, in North Maluku). Could AI have come to Manado from Ternate for treatment? Visiting from Ternate?
I found a page of maps of the Minahasa region, with cities on one and (color-coded!) regencies on another: (http://tinyurl.com/k3slv) where you can see the area of Manado; it abuts the Regencies of Minahasa and North Minahasa. Rumoong Bawah (Low) is in South Minahasa, very close to Amurang.

Human-to-human transmission possible in Indonesia’s 65th avian flu case
Sep 14, 2006 (CIDRAP News) – The World Health Organization (WHO) today recognized two cases of H5N1 avian influenza in Indonesia, including one from March that was first reported yesterday by a WHO official and one from May involving a man who may have become infected after exposure to an ill family member.
In both instances, the WHO has retroactively confirmed the cases on the basis of its new criteria for laboratory confirmation. The boy’s case put Indonesia’s count at 64 cases with 49 deaths, and the man, who recovered, represents the country’s 65th officially confirmed case.
The 5-year-old boy from East Bekasi in West Java province became ill on Mar 4, was hospitalized 2 days later, and died on Mar 19. The WHO said test results using two different assays revealed high antibody titer for H5N1 on serum samples taken on days 11 and 15 of his illness. Field investigators had determined that the boy had been exposed to sick poultry near his home, where some birds had tested positive for the H5 virus subtype.
Investigators identified the second case, in a 27-year-old man from Solok in West Sumatra province, when they traced contacts of the man’s 15-year-old sister who had a confirmed H5N1 infection in May. Her brother spent 6 days caring for her during her hospitalization. On May 28 he experienced mild cough and abdominal discomfort but no fever. His symptoms improved and he recovered in a few days.
Despite his mild and atypical symptoms, the brother was tested as part of the health ministry’s contract tracing and management program. He was given a 5-day course of oseltamivir beginning Jun 1 and was placed in voluntary isolation during his recovery.
Initial samples were negative for H5N1 infection; however, in August, follow-up testing of paired-serum samples found a fourfold rise in neutralization antibody titer for H5N1, a result that meets WHO’s new criteria for laboratory confirmation.
The man reported no contact with diseased or dead poultry before he became ill. Investigators determined that human-to-human transmission resulting from exposure to his sister could not be ruled out.
The WHO issued a report confirming his sister’s H5N1 infection on May 29. It said she remained hospitalized, but no details were available on the outcome of her illness.
In other developments in Indonesia, the director of the country’s main treatment center for avian flu told participants attending a scientific conference in Jakarta yesterday that avian flu will continue to circulate among the country’s poultry flocks because of shortfalls in vaccination and biosecurity measures, according to an article today in The Jakarta Post.
Santoso Soeroso, director of Sulianti Saroso Hospital, said improper vaccination of ducks and chickens may be contributing to the spread of the H5N1 virus. He said authorities are using substandard vaccines and are unable to evaluate the effectiveness of the programs because they lack an unvaccinated control group.
Soeroso said recent research in Bali province revealed a vaccine failure rate of 39% and that 60% of the area’s ducks had avian flu.
Indonesian agriculture minister Bagoes Poermadjaja told the group that only 60% of the country’s 300 million chickens and ducks had been vaccinated. He said the country lacked resources of early detection and is not adequately compensating citizens for slaughtered birds.
He said biosecurity measures are also deficient, and he noted that most cases of avian flu in humans had occurred in provinces where transport of poultry is constant. Poermadjaja also said weak coordination between central government and regional administration hinders the coordination of avian flu prevention and management.

Just looking at the format of the chart - want to make sure I understand - of the 77 suspected cases:- 18 died plus 5 tested positive = 23 total - plus 34 tested neg = 57 total - so where are the other 20? Waiting for test results? no word on them? or is that the wrong way to interpret the chart?? Double entry bookkeeping was never my forte obviously!!

gharris the columns tell you what month the cases were in and those totals are at the bottom. If you want to know about the type of cases then read the rows from left to right-those totals are at the right. the bottom , right number gives you the grand total and you can see it adds up either way-left to right or up and down. there were a total of 134 cases found in the news-of those 9 died and they had no testing done, 9 died who tested positive, 5 others tested positive , 77 were suspect cases and I guess tests are pending etc., 34 (of the total, not the 77 ) tested negative -add them all up and you’ll get 134. Hope I did not confuse you even more;-)

Gharris, thanks for taking an interest. Worrywart’s understanding of the confusing information is accurate. I’ll change the description on the chart for the line called “Suspected symptoms” to “Symptoms, tests pending.” No one is supposed to be double counted, and the total at the bottom is the grand total.
I changed the description on the first line to “Died, no test results” to encompass even those who have died with tests, but the test results are not yet known. I think only one person in that category had tests done, so if the results become known, either the “Tested negative” or “Died, tested positive” tracking will go up.

Regarding the 27 year old male, no name was given, but he is from Solok, W. Sumatra, and was ill back in May (before our tracking period.) See the WHO update 31 for their summary.

I haven’t been able to do much since Thursday night. Friday my dog was diagnosed with canine diabetes and is still in the animal hospital. I have been trying to use my time to study this disease and to get used to the idea of the injections. I also now have to figure out how to prep for the insulin we will now need. I may not be getting anything done with the Indonesian thread for awhile either until we get this situation under control.
I will still try to do a little searching as a break once in a while when I can. Everyone is doing a great job finding news BTW. Sometimes there isn’t a lot to find, lately some of the translated names have been interesting reading too:)

BRMM—we are happy you are here to help. I was supposed to be doing Cikelet, but there has been no real news out of there for awhile, so I just scan all the articles to see what I can find. I think Sumatra and Sulawesi and “other” have been officially vacated, but it doesn’t matter, you just post what you find. Anything and everything is appreciated. Some people have favorite newspapers and might catch something that another doesn’t catch. We are all helping each other. I really don’t know how long this exercise will go on. Not because I won’t want to do it any longer, but because, at some point we will all have to evaluate the inherent value of doing it. To extrapolate, what if we find another 100 cases, or 200 cases or 300 cases?? I don’t think the WHO will change their position or the world will take any more notice. I guess why we are all doing it, is because we want to be the ones that have the very first indication that is has gone H2H. We all will have that first “gut” feeling that it has ignited and you won’t hear about it anywhere else. Believe me, I have been close to that feeling. So I will keep on with my feverish looking for info, and I think we all just have to keep sharing with each other the relevance, (or lack therof,) of what we are doing.

Also, I know some people do not want long articles on this thread, but it is helpful to know you are working from the available information rather than go back and re-translate the entire article.

I have looked at a large number of my Indonesia news sources and there is hardly anything mentioning bird flu. The only exceptions seem to be articles about future poultry vaccinations or future funds for vaccinations, or a few culling operations (see pugmom – at 12:18).
All of a sudden there is silence where not long ago there was a huge number of articles all over Indonesia. While part of me wants to say, “Hey, this good news, all is calm in the world of bird flu.” But there is another part of me that wants to say, “Hey, somebody has gotten orders to stop reporting flu burung news or else.” Has there been a news clampdown? He said some 20,000 poultry had been culled in the five endemic areas during the past three months. “Poultry deaths in the five areas from the H5N1 virus were quite rapid and have become a cause for concern,” he told The Jakarta Post.
The office has distributed 500,000 ampules of bird flu vaccines provided by the central government to curb the spread of the virus.
“The central government has provided 2.5 million ampules to the province, but we have only distributed 500,000 of them. We will distribute the rest in a few days,” he said.

Commonground at 7:11, sometimes you find sites that don’t allow you to just copy and paste- very frustrating I know. One thing I’ve tried that will work with some of them is to go to your top bar on your screen and right click on ‘edit’, then click ‘select all’, then click on edit again, then click on copy. Open an empty ms-word file and paste the whole page there, copy what you want to translate from there in the usual manner- if you can’t post right away make sure you copy the link too. If you go back over the news reports, the slow down in news on suspect cases seems to go hand in hand with announcements of world bank and US funding for the region to fight Bird Flu. Anyone else think there may be a connection? It also goes hand in hand with a great increas in reports of measures being taken to get BF in birds under control in the region with culling.
An increase in BF news appears to cause stock market and economic instability. Therefore, it is the vested interests of global economic stability to keep BF out of the news as much as possible. The money to fight such things is provided by those organisations that seek to maintain global economic stability.
Personally, I think that is what the silence is all about, because, irrespective of what we learn on these sites about the progress of the H5N1 problem, it does not change or lessen the measures being taken to counter the problem on a global scale. Therefore, I think that TPTB think it is in everyones interests to keep a lid on the news of human infections (and so reduce the subsequesnt economic jitters on world markets) as much as possible.

I tend to think that the news shutdown has been at the testing point in the laboratories (I assume government funded). We haven’t heard of any ‘new’ positive confirmations in a very long while.
Because of this thought, I also have a question… Do you all interpret the two new ‘negatives’ as laboratory confirmations, or as statements from the hospital. I tend to think they are from the hospital saying “They were suspected of having bird flu, but now they are well enough to go home because they don’t have the flu anymore.” That is supposed to be the result when patients get on Tamiflu.
If this is the case, I feel we should continue to show these individuals as suspected cases instead of negatives. Please speak up if you think differently.
I’d have to agree with Michelle about the lack of news regarding human cases. If the labs are goverment funded and the goverment doesn’t want the news released publicly, then likely it won’t be.
OTOH the switch to doing the testing locally only is quite recent and it’s possible the info can’t be released until certain departments in the goverment have been ‘properly’ advised- depending on how that is set up there could be a considerable amount of red tape involved. The delay or lack of current lab results could just be a product of inefficient bureaucracy (redundant? :-)
Michelle in OK, my vote would be that we don’t label people negative until lab confirmation occurs. Clinical diagnosis might send them home, but test results are definitive.

I posted something on the News thread on how Indo is feeling the economic impact of this. I agree with flumonitor. And then I agree with Michelle in OK because the hospital are funded by the Government.
The UN special envoy for avian influenza, David Nabarro, met last week with senior government officials and discussed the problems fighting the disease in the country. He outlined the issues at hand in an interview with The Jakarta Post’s Hera Diani.
Question: What was the core of your discussion with Coordinating Minister for the People’s Welfare Aburizal Bakrie?
Answer: This is my fourth visit to Indonesia within a year and I have been working closely with your government to examine ways in which avian influenza in both birds and humans is being tackled. In each visit, I want to find out how the government’s response is improving and I’ve been really very pleasantly satisfied with what I see as a real improvement in the last few weeks, particularly with the way in which the avian influenza is being tackled in poultry.
The discussions with the minister were about the plans for extending this animal health work to other districts, about the government finance, about the compensation payments and how the government is proposing to increase the compensation.
We also talked about the vaccination program for birds and the public information campaign. We agreed to create a detailed plan with an action checklist undertaken by different stakeholders in central government and also at the district level. We will look harder at the resources needed and we will try to make sure that the donor communities respond to the promised increase in government funding with an increase in donor funding as well.
Indonesia’s handling of bird flu has been widely criticized. Why do you say you were satisfied?
Well, first of all the changes that I’m describing have occurred quite recently. Secondly, the challenges faced by Indonesia are very big, and it takes time to put in place some of the planning and early development. And thirdly, I’m satisfied that there has been progress, but at the same time there’s a lot to be done, particularly by the agriculture ministry. It is important for somebody in my position to speak out about good progress. At the same time, I must stress that there’s a lot more that needs to be done to get good quality animal health services in all 444 districts in this country.
Could you elaborate in which areas improvements should be made?
The following activities are necessary. First, it is vital that veterinary personnel in each district are looking out for bird flu cases and responding to them. That means the efficient killing of affected birds and those around them, and reliable vaccinations to try to reduce the spread the disease among birds. It also means compensating those who have lost their birds and making sure that the way the birds are cared for reduces the risk of infection in humans.
These are quite difficult activities to introduce, especially because the political system in Indonesia is very decentralized. We need to increase the level of activities at the provincial or district level. And we know that it is difficult work, so from outside we’re expressing solidarity with the government and the people of Indonesia.
The National Committee on Bird Flu Control and Pandemic Preparedness recently said it had not received a single cent from donors. What is your comment?
I think today we were able, in a careful way, to look at the government funding for avian flu and also the way in which the compensation and other activities can be funded. It is clear that there are ways to increase the funding provided by government. I won’t go into detail because you have to ask the government about this.
There has also been a lot of funding for Indonesia. It has not all gone exactly where people might be expecting it, and so we have been discussing ways in which the funding will increase. Not as much as we want, but there are perhaps more than US$40 million of additional assistance that is going to be provided in the next few weeks and months.
We’ve been planning to make more efforts to mobilize even more money from the donors. I need to go to these donors and say “look, Indonesia’s program has improved in the last few weeks, so you should put in more because the government is ready to use the money well.” It is a continuous process of mobilizing money; you don’t do it once and it requires the donors’ understanding of what is happening in the country.
The government, on the other hand, has to demonstrate that the money it has is being used well, that there is a need for new money, and it has to have system to spend the money efficiently and without any losses. These things have to be in place to raise cash. You have to have good program and you have to demonstrate it’s good, and then the cash will come.
The donors are said to provide funding in the forms of goods and technical assistance instead of money. Is that deliberate?
The avian influenza problem in here is very complicated. And so one of the first requirements is for technical specialists, both Indonesian and international ones, who can study the nature of the problem, and then understand its severity in order to plan the control strategy.
You can’t actively research a strategy without having technical people working on these issues. I’m very pleased that there has been a strong technical focus recently. I see that there is a lot more technical expertise here now than there was some months ago.
Is bird flu here getting worse?
One of the things that happens when you improve the surveillance of the country is that you also likely to report more avian flu because you’re finding more cases. So, at the moment we’re certainly have information that tells us that avian flu is quite widespread in more than 290 districts out of 444.
