You know, as I read Niman’s repeated posts on the likelyhood of little connection between H5N1 human cases and bird cases since the virus differs, it is hard to not imagine we just have NO IDEA what is going on there. I CAN imagine easily dozens of cases unreported for every reported case. In particular, the unreported cases could be steadily increasing with no clue to that occurance. I just don’t find the repeated “sick birds in the area” comments to be reassuring, in fact the opposite!.
Is there ANY testing occurring on flu symptom deaths AWAY from sick bird H5N1 outbreaks? How tight is the connection. Their cultural relationship with hospital care would really point to the idea that people could be dying at home regularly, and if the regular medicine people aren’t canvassing other than the areas of sick birds, no “officials” would know or care!

Comment: Cottontop - as with the most recent cases in Jakarta of MK and the 11 year-old, there are often birds in the area of where a human infection has been found. Both of these families adamantly insist that there were no sick birds found recently in the area of their homes. The tie to the birds is inconclusive sometimes, but they cull the nearby birds anyway just in case.

Samples taken from a 67-year-old woman who died overnight and an 11-year-old boy who died two days ago returned positive results from two laboratories, the health ministry said….
The bird flu centre’s Rusin also said that another patient, whose details he did not provide, had returned positive results to an initial test but the results from a second laboratory were not yet available.

The Ministry of Health in Indonesia has confirmed an additional three cases of human infection with the H5N1 avian influenza virus. All three cases were fatal.
The first newly confirmed case occurred in a 67-year-old woman from West Java Province. She developed symptoms on 3 October, was hospitalized on 7 October, and died on 15 October. Diagnosis was complicated by the presence of chronic diseases. Chickens reportedly died in her household and neighbourhood prior to symptom onset.
The second case was an 11-year-old male from South Jakarta, Jakarta Province. He developed symptoms on 2 October, was hospitalized on 5 October, and died on 14 October. His recent history included exposure to dead chickens in his neighbourhood.
The third case was a 27-year-old female from Central Java Province. She developed symptoms on 8 October, was hospitalized on 12 October, and died on 13 October. The source of her exposure is currently under investigation.
Of the 72 cases confirmed to date in Indonesia, 55 have been fatal.

One, consisting of three patients infected in 2005 map to the upper branch of the bottom of the phylogenetic tree presented at a WHO meeting in Jakarta in June. The vast majority of Java cases however, map to the lower portion of the tree. All human sequences that have been made public since the meeting, also map to the lower portion. This lower portion has no avian isolates.
The match failure was noted at the meeting, and 91 recent samples were sent to a WHO affiliated lab in Australia, Over 50 bird sequences have been made public from these recent samples and only one bird isolate from Java had the cleavage site. However, it match the small group of three patients from 2005. This duck isolate was from Indramayu, but the patients from Indramayu mapped with the majority of cases, which are on the lowest branch.
Two chicken isolates did map to the lower branch. By these isolates were from 2005 in Central Sumatra, far from the human cases on Java, which have stretched from East Java to West Java and isolated during a time from from the index case in July, 2005 through the most recent deposits, from the summer of 2006.
The origin of these infections is unknown. The only non-human Java match was from cat, also from Indramayu. That sequence was closely related to the human sequences, including those from Indramayu.

“we had a finger problem”? Perhaps this is another one of those phrases that just doesn’t translate from Indonesian into English, or maybe they are using their proprietary software to translate their reply?
Comment: Updates on MK (67) and Doni (3).
MK’s body was buried right away, and it sounds like no autopsy was done in spite of the confusion over whether she was H5N1 positive, and in spite of the unusual affects the virus had on her brain and other organs.
Doni seems to be doing better, but this article notes that he had contact with birds. His mother, in another article, stated uncategorically that he did not have any contact with any birds or other animals.

I don’t think they are close. Sigedang is almost on the north coase of Java. Kaliurip is in the middle of the land-mass of Java.

In my searches, I found out that Sigedang is home to a huge artesian underground spring and it has been bought by a private company and tapped out and bottled up. The women villagers there have formed a consortium to try to break up the monopoly that is bottling all the water, as the farmers no longer can irrigate their fields and the drinking water is running out, as they have depleted all the underground water. I think that as time went on, we may have expected more clarity in H5N1 testing and swift handling of suspect patients. Instead, things seem to be moving the other direction, and confusion is building along with what seems like a continual flow of new cases and deaths.

JAKARTA (AFP) - The death toll in Indonesia from bird flu rose to 55 after officials confirmed that a 27-year-old woman who died last week was infected with the virus.

“Both tests showed her to be positively infected with the bird flu virus, making her the 55th fatal casualty” in Indonesia, said Tontro, an official at the health ministry’s national bird flu information centre.
Positive results from two Indonesian laboratories mean that the World Health Organization (WHO) includes the case in its records.
The woman from Central Java province, identified by the centre as Mistiyem, developed symptoms on October 8 and was hospitalised on October 12. She died a day later, according to a statement posted on the WHO website.
The WHO said the source of her exposure to the virus was currently under investigation.
The vast majority of cases in the archipelago nation have been spread by contact with infected poultry, though several cases of limited human-to-human transmission have also occurred here.
More human cases of the virus — 72 have now been reported in Indonesia, the world’s fourth most populous nation — increase the possibility of the H5N1 virus mutating to become easily transmissible among people.

The plague of dengue fever raged in Pontianak and the other area in West Kalimantan as far as Ahad (15/10).

There seems to be no resolution of over a 100 suspect cases. There have been no epidemiology reports/analyses. This virus seems to have free-reign to experiment and mutate to its heart’s desire. We have had no analysis of the effectiveness of the Tamiflu blanket, which initially was a pretty nifty idea/slogan, but now just seems as an act of desperation. We have no idea what the vectors are. We seem to be Neanderthals in our understanding of it. My only question is will it erupt this winter or next winter. In my hopeful moments, I am able to envision a scenario where the clusters grow slowly and percolate along, and the explosion will not take place till the winter of 2007–2008. That is the ONLY question in my mind—whether it happens this winter or next. We still haven’t seen the real monsoons, of which the Indos are so afraid of, as they know viral virulence increases greatly. Those rains are coming soon. And, once again, to you nay-sayers, I ask you to do just one thing---go to the Wiki and print out the case summaries that Michelle, et al, have been tracking since June. Print that out and then, after analyzing it, make your own determination about just how close we are to PanFlu.

Actually, even with that evidence, no one can say how close or how far we are to PanFlu. Nay-sayers or not, no one knows at this point, as frustrating as that is.
We can all envision scenarios, both horrific and quite pleasant - but the virus will do what it will do regardless. All we can do is keep a close eye on it. How you react while we watch it is up to you. So far, the nay-sayers have been right. It doesn’t mean they will always be (and if they are not prepping they are losing some very good opportunities).

Tests on the woman from Central Java province showed she had the H5N1 strain of avian influenza when she died last week, the WHO said in a statement yesterday. A 67-year-old woman from West Java province and an 11-year-old boy from a southern suburb of the capital, Jakarta, also died from the disease.
The virus is reported to have killed a person every four days this year, more than double the 2005 rate, creating chances for it to mutate and spread easily among people. Pandemic flu could kill millions, and nations should decide in advance which groups of people would receive drugs and vaccine first, world health officials say. Almost a third of nations surveyed prioritized neither in their preparedness plans, researchers say.

Interesting, though, that I do not find the article in PLoS Medicine that is referenced in the Bloomberg story. PLoS Medicine provides information to PubMed in advance of print publication (at least some of the entries in PubMed have that notation) but there are only two PLoS Medicine articles that have reference to H5N1; one in August of this year (Live, Attenuated Influenza A H5N1 Candidate Vaccines Provide Broad Cross-Protection in Mice and Ferrets) and one in September (Reducing the impact of the next influenza pandemic using household-based public health interventions). Neither article was authored by Uscher-Pines.

You know, as I was reading your detail above about the shamans, of course my blood pressure and frustration level went up.
But then I began to think about how many physcians here that I have spoken to recently just do not “believe” in a pandemic. Each one has smiled in that way that says that they know better than this layperson. No physicans here will prescribe antivirals or antibiotics in advance of the imagined pandemic for their patients who they took an oath to protect. I shake my head each time I encounter this response from the medical community here, and it has made me realize that it is not only the Indonesians who have “beliefs” that are preventing lives from being saved. Our university trained Western doctors do too.

September 06, 2006 Dealing with Dengue Fever: Indonesia… Dengue fever is a relatively new disease, appearing initially in the 1950s in Southeast Asia and causing just a few thousand cases in that decade. The infection has since spread to most tropical and subtropical areas.
During the 1990s there were nearly 200,000 cases in all of Southeast Asia. In just the first half of this decade, there have been over 300,000 cases in Indonesia alone. The dengue problem grows steadily worse. There is no drug that prevents or cures infection. There is no vaccine. If you get dengue fever, you ride it out…
Last year in Indonesia there were about 80,000 reported cases, and nearly 1,100 deaths caused by dengue. Most experts consider the disease underreported by a factor of two or three — one reported case for two or three actual cases.
Most people infected by dengue experience mild-to-severe discomfort, typically a headache (especially behind the eyes), malaise and maybe a rash. But about 1 to 2 percent develop a more severe form of the disease called Dengue Hemorrhagic Fever (DHF).
The case fatality rate for DHF ranges from 1 percent to 40 percent, depending on promptness and quality of supportive clinical care. Dengue is a fact of life in Jakarta, with thousands of cases and dozens of deaths each year, and most cases occur during the rainy season….
Dengue is transmitted by a mosquito that is abundant in Jakarta, Aedes aegypti (and a sister species, Aedes albopictus), the Asian Tiger mosquito. It loves the rainy season. The Asian Tiger bites during the day… When you see insecticide fumigation going on, this represents a sure sign of failure to effectively control breeding sites. Fumigation effectively knocks down the adult mosquitoes that happen to be present when it occurs, but many resting mosquitoes survive and can safely take to the air just a few minutes afterward.
Aedes mosquitoes can be born with dengue virus inherited from their mothers. That’s right — a mosquito hatched in your yard and never going anywhere else can transmit dengue fever….

Thanx Okieman—it appears the Dengue outbreak started last July and is still going on. After having digested all these posts and writings on Dengue in Indo today, I think I will be slightly less worried when I hear about the “thousands” of cases occurring, whether in Kalimantan or Jakarta. But that is good, as I don’t really want to get too distracted—tracking H5N1 is hard enough.
Comment: More on Doni here (Dn), perhaps, who is reported as being (5). They are diligently looking for a link to birds with AI, but report that Doni seemed to have had no contact with infected animals.

CG: I think you know more than you think you know. It has been commented on that ducks are typically carriers, but do not display symptoms or deaths. Maybe this is another sign of the virus breaking out, or gaining virulence in other species.

About domestic ducks in Thailand, the PNEAS paper said:
“As previously mentioned, our laboratory received 450 sick, moribund, or dead ducks, which were studied for pathologic features of H5N1 infection. These birds had been raised in the open house system or were from backyard flocks. They exhibited signs of disease such as high fever, dyspnea, depression, and diarrhea, and nervous signs such as ataxia, incoordination, and convulsions (Figure 3A). Most had ocular and nasal discharge accompanied by conjunctivitis; 20%–100% of the birds in each flock from which these ducks originated were dead. All cloacal and tracheal swabs and tissue samples were positive for H5N1 by HI and RT-PCR (results not shown).
At necropsy, gross lesions were detected, including ecchymotic or petechial hemorrhage of leg and footpad; serous fluid surrounding the heart, pancreas, liver, and abdomen; cyanosis of the oral cavity; and mild pleural effusion. On histopathologic examination, the most striking lesions were found in the lung, with extensive pneumonia and severe pulmonary edema with hyaline material in the alveolar space and slight mononuclear infiltration in the area surrounding congested vessels (Figure 3B). Nonsuppurative encephalitis with perivascular cuffing of mononuclear cells and gliosis were detected in the brains of ducks that displayed nervous signs. Hyaline degeneration and necrosis of myocardium with mononuclear infiltration were detected predominantly in dead ducks from fast-growing breeds such as the Pekin and white Cherry Valley ducks. Necrotizing pancreatis with mononuclear infiltration was detected in all affected ducks. Most affected ducks exhibited focal hepatitis, tubulonephritis, splenic lymphoid depletion or necrosis, and enteritis. Virus antigen was detected by immunohistochemical tests in all organs tested, including trachea, lung, liver, pancreas, rectum, bursa of Fabricius, spleen, brain, heart, and kidney (Figure 4).”
It used to be thought that ducks were asymptomatic carriers of influenza viruses, but when H5N1 appeared on the scene, that pattern no longer held true in all instances. There are now numerous sequences of H5N1 from ducks in the databases. While the records don’t always state whether the sample was from a dead or live duck, I remember from old news reports that at least some of them were dead.
Some of the locations include China, VietNam, Novosibirsk, Egypt, Italy, Bavaria, Iraq, various locations in Africa, etc. As for Indonesia there were locations in Bali, and at a place called Parepare. There may be more locations, I did not look through everything.
There is probably a serious lack of testing on ducks (as is true with mammals). But I find it hard to believe that most of the sequences were from live ducks, quite the contrary, they were probably from sick or dead ones. While there is significant monitoring of healthy ducks in North America and probably some locations in Europe, that’s probably not the case in many other countries due to lack of funds; therefore in certain areas of the world it would be reasonable to surmise that samples came from dead ducks…

There has probably been a certain mortality rate in ducks from H5N1 all along. But the thing that caught my attention in this article about the suspect case who tends ducks is that the duck-death in this one little area is a new phenomenon for that area. The “Tim” has been sent out to investigate this new occurrence. It may be things like this, that is, miniscule little changes in viral behavior, that will give us the heads-up and show us the slowing evolving trend lines.
Oh Yeah, and I forgot one other thing—remember Dr. Niman and that now famous Indramayu duck—the only thing that matched a human sequence. (I think I have that Indramayu duck burned into my brain.)

I’ve been doing some cross referencing lately against our spreadsheet. So far, so good. Regarding the 2 month old above, Fajrin, I was surprised to see 7 people in Maros on August 29th. All were given Tamiflu, Tested and gone home. Except for the last person whom was not tested, but was given Tamiful and gone home. Sounds like a Tamiflu blanket if you ask me. LONDON, England (UPI) — In a week that has seen three deaths from avian influenza in three days, all in Indonesia, the country defended its efforts in the battle against bird flu.
‘We are doing the best we can,’ said Nyoman Kandun, a senior Health Ministry official.
While it has been established that the best way to eradicate avian influenza in poultry is by mass action, be it culling or vaccination, the Indonesian government says that it is unable to offer sufficient compensation to the country`s millions of backyard poultry farmers. The sheer number of domestic flocks in the country makes any vaccination program costly and unwieldy.
Last month the Indonesian government launched a bird-flu information campaign, teaching people how to avoid contracting the disease through proper hygiene and due care. But the recent spate of deaths — Indonesia has suffered the highest avian-influenza toll of any country — seems to indicate that such programs are not working.
On Friday a 27-year-old woman from Central Java died a day after being hospitalized with flu-like symptoms. Her death was confirmed Wednesday by the World Health Organization as being directly attributable to avian influenza — Indonesia`s 55th such death.
Then Saturday an unidentified 11-year-old boy died in Jakarta`s Sulianto Saroso Hospital for Infectious Diseases.
On Sunday a 72-year-old grandmother died of avian influenza in a highly unusual case that also saw her affected by encephalitis. The woman, from Cisarua in West Java, was placed in a bird-flu isolation ward Oct. 7 and lost consciousness for a day due to encephalitis. Her kidneys were also affected.
No other Indonesian bird-flu victim has been affected by encephalitis; the only other reported case occurred in Vietnam in 2004. Encephalitis is an inflammation of the brain that is associated with viral diseases, bacterial diseases such as bacterial meningitis, or complications arising from diseases including syphilis and rabies. Avian influenza, on the other hand, traditionally attacks the lungs.
“ The virus in her was highly pathogenic, very vicious. She is the 54th casualty out of 71 cases,” said Runizar Ruesin, the Health Ministry`s bird-flu information center chief.

I don’t think I am a finicky parser, but I didn’t and don’t believe MaMa was the first person with encephalitis. That is just my opinion and I am not a medical person. To me something ended up messing up their brain period. I don’t know for sure THEY know either or if THEY do they aren’t telling. I do know that Jonas Ginting had an abcess of some kind and I have this from the article from when he came home.
He could also experience brain access in his head because of the bird flu attack. That was ‘known after doing the inspection with CT Scan so as to be seen by the pus collection in the Jones brain part. But, because kedisplinan and Jones diligence to take medicine, finally he was free from the bird flu illness and brain access and only was kept experiencing the pulmonary disease. So, Jones will be appointed to take medicine the road a week very much up until 6 length months.

Jonas was younger—probably what saved him. But the way they wiggle out of confirmation of H5N1 invading the brain is to use very precise medical terminology. You can have a brain abscess (pus pocket) with no encephalitis. Brain abscesses are usually bacterial in origin. Encephalitis is usually associated with virues (as Western Equine Encephalitis). Encephalitis is a general term meaning inflammation of the brain/and/or cerebro-spinal fluid. You can often diagnose the causative agent of encephalitis by getting at the fluid that surrounds the brain, ie a spinal tap. You cannot always diagnose what is inside a brain abcess by doing a tap, as a tap only tests the fluid that bathes the brain. You can see a brain abcess on a CAT scan though or by directly getting tissue from the pus pocket itself. So, if they never did a spinal tap on Jonas (and he would probably have to sign a consent form and we all remember he wasn’t too compliant), then they would never be able to say that there was H5N1 in his cerebro-spinal fluid (viral encephalitis), and if they never tested for H5N1 in the fluid, then it is like it never happened. And since he didn’t die, they didn’t perform tissue sampling on his brain. So, the old rule applies: if it wasn’t done, or it wasn’t written down, or if it wasn’t tested for and documented, well then, obviously it never happened. Sorry I rambled on so long. Maybe these are just glaring omissions by Indo doctors (fortunately, for us here in this great country, we would have a differential diagnosis of what was going on in the brain within several days—no stone would be left unturned.) I am not convinced that it is a deliberate cover-up by the Indo PTB, or just sub-standard care/knowledge.

pugmom - technically, we are still awaiting confirmation of MK’s spinal fluid tests. They were supposed to be returned on Tuesday. I have looked all over for them and there is no mention of these final test results anywhere (and since it is now Thurs. in Indonesia, I am beginning to think there won’t ever be any final confirmation). At first we heard that the tests of the spinal fluid came back positive. Then we heard that the tests had not been done yet. Finally they promised test results would be returned this past Tues.
The doctors at the hospital surely seemed like they were going full-out to find the cause of MK’s illness, quite apart from a bit of professional infighting that is not unusual in high profile situations anywhere. Since they seemed so diligent, I really did think they would follow up and release the final spinal fluid test results as soon as they were returned. I am rather puzzled that they have not.
I am even more puzzled by hearing from all of you that no final spinal fluid test results were returned for Jonas. That fact had slipped by me completely. Perhaps I had just read Niman’s strong commentary on that situaion and assumed that the final results had become known and I had somehow missed them. I always thought we had final test results for him too, until you ladies clued me in. Pugmom, your thoughts about whether this is all intentional or something that has just fallen through the cracks is worth pondering. (Especially after reading that PNEAS report today which stated that the H5N1 positive ducks also had encephalitis).
Comment: You have just got to love this title. But that is the only thing funny about this story: there are new cases, a new fatalilty, and a report of 25 suspected cases treated in the hospital in Makassar within the last three weeks.

I know you know what you are talking about concerning the brain abcess/encephalitis issue. I think sub-standard care is probably involved in a LOT of cases including misdiagnoses. I still think, just my opinion, that it is 1. Half the time they don’t know what they are doing over there or 2. When they do know, they aren’t going to tell the truth, they will spin the truth or hide it. 3. I have read too many articles about BF effecting tourism, just before their National Bird Flu Campaign was launched I read two articles about that very subject including mentioning $ goes towards BF not to line officials pockets, now that tells me something, so my personal jury is still out on whether anything is intentionally withheld by Indo PTB. But if I were to poll that jury today, I would say it is leaning towards a guilty verdict. Just my opinion anyway, I could be wrong. Time will tell. Get some rest catch you tomorrow.

Well, I don’t know if it is a “real” translation, but is is the ToggleText translation! It certainly gave me a chuckle. And it is almost like they are saying that THEY are loosing track of how many suspected H5N1 patients are being seen at Wahadin too! When they put out an article with the title saying that there were “a whole heck of a lot, we mean really a lot” of suspect patients seen, we are clearly in trouble.

Except I showed Fajrin as from Maros, not Parangtambung. In addition, I’ve added Asrianti from Maros and Beautiful from Parangtambung (I couldn’t find that location on a map… If it’s a subdivision of Maros, that would clear up the confusion regarding whether Fajrin and the Dawn are the same person.)
I don’t have the other new patient from Maros, Asr/USA/Asrullah (13),as entering RSU Wahidin Sudirohusodo but instead the Hospital(RS)A Makkasau.

I think it is important to continue with the detail at this juncture. In two locations, Bandung and Makassar, we are very close from what we hear to the WHO definition of Level 4 clusters - 25 people or more infected. Right now, the only thing that is preventing that from being true in fact is that we are seeing the cases spread out over weeks, rather than days (and the WHO says nothing particular about timing), plus many cases never seem to receive any definitive test results. It seems that if a patient gets better, they are assuming it is not H5N1, whether at a local community hospital or an H5N1 reference hospital.
We know that the information is not flowing out of Indonesia, and so must be ferretted out. Neither Indonesia nor WHO is enthusiastic about seeing this thing go to the next level, so I doubt there will be any clear confirmations from them. We are left, then, with the usual detective work, and I vote that we continue.
I’ll make up a spreadsheet to keep track also of the cumulative cases. You are right that there is a creeping realization that half a day away from the situation can leave us feeling that circumstances have outrun us, but maybe another person following along with spreadsheet detail will help to fill in Michelle in Ok’s official talley.

When I first started reading the post from Commonground – at 06:34, I was hoping maybe the 100% increased in bus passengers had something to do with the end of Ramadan.
The last sentence might explain that’s the reason.

Does it mean at first Agus thought people where afraid that they might not be able to get on the buses , but then he tells them if they had bought the tickets long before, they were guaranteed a seat?
And they refer to the passengers as returnees.
I hope I am correct in my assumption that it is Ramadan related. The last thing we need is wholesale villages jumping onto buses, fleeing a infected area, and into major cities.
In my mind it says: Passengers were concerned that there would be no (tickets) available, unless they had been purchased well in advance. JMO
JWB - at 09:58 - the reason I posted it was for just that reason. To watch for a possible fleeing. But in this instance, I think they were afraid they wouldn’t be able to get a ticket, they were afraid they would run out of tickets, they were not afraid of bird flu. But I go through 17 newspapers/websites each morning, and I don’t have time to scrutinize each story too deeply. I’m keeping my eyes out for any mass evacuations though. Buses, planes, etc. That’s why I posted the captiion under the very crowded terminal the other day.

Regarding the bus situation. Five Indonesian airports were closed this week due to the smog from their forest fires which have caused a terrible haze throughout the region. This would increase the number of bus passengers in a hurry.

JWB - Yes, it is just a busy travel season in Indonesia. They are about to celebrate the huge Muslim holiday feast of Id-Il-Fitr and apparently everyone (and they really do mean EVERYONE) goes back home to the countryside to spend the holiday with their families.
The holiday comes around October 24 - 26th this year, depending on which sect one belongs to. Everyone has purchased their tickets months in advance because there are reports that airplane tickets rise by 100% during this period, for example, and seats are scarce.
They are noted as “returning” because everyone is returning to their home villages. Apparently about 2/3 of Jakarta clears out at this time. Everyone will feast with their families, eat plenty of holiday chickens, let the kids play in the country barnyards, and then they will all travel back to the cities sometime after October 26th.
I forgot to resort by location, so the above sort is by date. It would have been clearer by location… my apologies. Also, the reference to Jakarta at the end of some lines is confusing so just ignore it. It belongs to the column for “Lab where samples were sent.” The n/a applies to some of the people who were never admitted to a hospital.

I don’t know about phone numbers, but e-mail would be pretty easy.
Just make up the e-mail in advance with all of the addresses entered and then save it instead of sending. Then when you want to send it just go to your folder with “unsent e-mails” and hit the send button for the appropriate one.
On AOL the folder is called “mail waiting to be sent.”
