Is Avian flu the next pandemic?

Apocalyptic mayhem, contingency plans, and gas masks have re-entered the rhetoric of some academic and medical professionals who are thinking seriously about a flu pandemic.

Educational efforts urging individuals to take action stress several problems that would arise in the event of a new pandemic.

Web site will help prepare for influenza pandemic

The Hamilton County General Health District has launched a pandemic influenza Web site, www.hamiltoncountyhealth.org/pandemicinfluenza, to help individuals, families, businesses, schools and other organizations prepare for a pandemic.

The Web site will be updated frequently and will serve as a clearinghouse of information for any member of the community who is interested learning more about how they can prepare for a pandemic.

Arkansas, USA

County prepares for Avian flu pandemic

The conference room at the Arkansas County Courthouse annex was packed from wall to wall Friday with a representative from nearly every organization on the Grand Prairie. The diverse group’s homework was to devise a disaster plan should the Avian flu — or better known as the “bird flu” — hit home.

“It is estimated that about 50 percent of people in the U.S. will contact the Avian flu if it makes it here,” Arkansas County Judge Sonny Cox said. “What scares me is that means that 50 percent of our staff will be affected. We will have to operate with only 50 percent of our people.”

The team was called together as part of the Arkansas County Office of Emergency Management who was given orders to have a team of leaders in place and an emergency plan to follow.

The worst-case scenario

National and international health officials have for the last year been warning of the danger of a flu pandemic that could infect billions of people around the world and kill millions. One of the greatest dangers such a pandemic poses is that the sheer size of it could overwhelm healthcare and other systems with days of the outbreak, leaving most victims to fend for themselves as society collapses around them.

“There is no doubt our healthcare system will be overwhelmed” in case a flu pandemic hits Arkansas County, County Judge Glenn “Sonny” Cox told a group of about two dozen healthcare, education, municipal, law enforcement and other officials during a meeting on flu preparations in Stuttgart last Friday. “There’s no doubt our law enforcement system will be overwhelmed.”

In the past, local law enforcement officials have been able to look to the Arkansas State Police of even the Arkansas National Guard in case of emergencies. That won’t be the case this time, officials say.

“What we’ve found in the last couple of years is ‘don’t count on it [help from the Guard]’ ” Neal said. “If they can be deployed, they will be in the urban areas. The rural areas will be on their own.”

Arkansas County Sheriff Allen Cheek agreed. The Sheriff’s Department has “10 fulltime deputies right now. If a pandemic hits here, I feel sure I will have to deputize some people — maybe a lot of people. Those people are going to need training.”

City, school and county officials will conduct an emergency drill at Westwood High today to test how prepared Mesa is for a flu pandemic or widespread illness.

As students leave school, they will fill out questionnaires and receive bags with emergency information and a snack.

Thirty more patients of Viral Hemorrhagic Fever were admitted in private and government hospitals in Karachi during last 24 hours. Forty-nine patients of the disease were discharged from hospitals after recovery.

Government and private hospitals in the metropolis have discharged 49 patients of the Viral Hemorrhagic Fever during 24 hours, while 30 new cases were admitted in hospitals, focal person to the Dengue Fever Surveillance Committee Dr. Abdul Majid told Geo News.

Provincial Programme Manager of the Sindh Blood Transfusion Authority Dr. Zahid Ansari has said a kit used in dengue fever diagnosis process need 96 blood samples and tests can be made after collection of the required number of samples.

Good morning, DennisC.

I see what you are trying to do with stories regarding Pakistan and Thirty more patients with Viral Hemorrhagic Fever. I know a lot of wikians have been concerned with India, Nepal, etc. and are watching it very closely. That’s terrific.

That said, at this point it has nothing to do with bird flu - no matter how much many think that is what it “might” end up being. I am concerned about the news thread becoming a repository for everything that “looks like it coould be bird flu.”

Would it be more appropriate and would we all be better served by a different thread where everyone can post and comment on illnesses they “think” “might” be bird flu?

“In the past few weeks, the Beijing Genome Institute has released a series of complete H5N1 sequences from poultry, wild birds, and swine in China. Most of the sequences are from isolates collected between 1997 and 2005 in northeastern China.

Of interest are two H5N1 swine sequences from Guangxi and Henan provinces. These sequence demonstrate the transmission and transport of H5N1 genetic information into the Qinghai strain, which has now been transmitted and transported to most countries in Europe, the Middle East, and Africa. This strain is recombining with wild birds and poultry in China and swine appear to add additional selection and acquisition of mammalian polymorphisms.

The swine sequences can be found in dog, cat, and wild cat isolates from Thailand, as well as human isolates in Thailand, Vietnam, China, and Indonesia. The movement of pieces of genes into Indonesian patients, including those on Java as well North Sumatra (the Karo cluster) is striking, and provides additional insight into the evolution and adaptation of H5N1 in mammals.

Similarly, Canada and the United States have now isolated H5 from wild birds in 2005 and 2006, but thus far only 4 of 8 sequences from one H5N2 sample, collected over a year ago in August, 2005 in British Columbia has been released. It has evidence of recombination and acquisition of North American swine H1N1 sequences and Asian H5N1 Qinghai sequences, demonstrating rapid evolution of low path H5 in North America.

Full sequences of the recent H5N1 isolates in Canada and the United States would provide information on the source and rate of this evolution and provide more information on the false negatives recently announced by the USDA.” - excerpts

Oct 18, 2006 (CIDRAP News) – The US Department of Health and Human Services (HHS) has issued new guidance calling for stronger respiratory protection for healthcare workers in the event of an influenza pandemic.

A new interim guidance document says the use of N-95 respirators—designed to stop 95% of small airborne particles—is “prudent” for medical workers providing any direct care for patients ill with confirmed or suspected pandemic flu and is recommended in caring for those with pneumonia. It also says respirator use is prudent for support workers in direct contact with patients.

In contrast, HHS’s pandemic influenza plan issued last November recommends that healthcare workers wear simple surgical masks, designed to block large respiratory droplets, for routine care of pandemic flu patients. Both the pandemic plan and the new guidance recommend using an N-95 respirator or similar protection during procedures likely to generate airborne infectious particles, such as endotracheal intubation.

The new document also advises healthcare facilities to expect and plan for shortages of N-95 respirators and similar protective equipment in the event of a pandemic.

The new recommendation reflects increased concern about the possibility of airborne transmission of flu viruses, though the document says the Centers for Disease Control and Prevention (CDC) has found no new scientific evidence on the question. HHS says the new guidance “augments and supersedes” the advice in the pandemic flu plan.

comment: I’m with witness on this one - there are soooo many threads, and traditionally we have been tracking other illnesses that present similar symptoms to H5N1, particularlyy if it is an area already known to have H5N1. Sometimes the news thread is the only thread I have time to check, and I am worried about other similar diseases that could be mis-diagnosed as something else. It happened in 1918 and could happen again.

Here is my tuppence worth - The News thread should be a repository of stories that are primarily BF BUT, as already occurs, many post to the relevant thread and to here so it gets caught in the main news thread. I think this is good - if people want to read in more detail they can go to the relevant thread. As I prepare the Summary, if I know there is a separate thread, I link it through there.

What we don’t need is for this thread to become a duplicate discussion of the dedicated threads but, none-the-less, as many wikians use the news summary and thread to get a ‘feel’ for what is going on, having the other info is valid - provided it is maintained at a ‘lower’ level. Some post along the lines of “I have just posted to xxx thread that yyyy and here is the link”. I think that is good.

As we have no idea where the BF pandemic will come from, or what will be the vector, keeping all our eyes and ears open is worthwhile.

There you go - my tuppence worth ;-)

State veterinarian says epidemic appears to be unlikely for now

BY FRANK RADOSEVICH II OF THE JOURNAL STAR PEORIA - For the time being, substantial scares of a central Illinois avian flu pandemic appear to be something that’s simply for the birds.

“I don’t think it’s very likely,” Dr. Mark Ernst, an Illinois state veterinarian, said regarding an epidemic. “At that time, a lot of people were getting things confused over a pandemic flu and avian flu.”

During the summer, some officials worried that migratory birds flying south for the winter across the Illinois River waterway could carry influenza viruses, commonly referred to as bird flu.

Those coming, and potentially infected, flocks circulated concerns about a regionwide pandemic which could possibly infect the local bird and human population, throwing the area into chaos.

However, according to city and state officials, none of those fears to date have come to fruition.

Pandemic Flu Detection System ( Air Date: 10/19/2006 ) Nebraska has a new system to fight the pandemic flu. It`s a computer system that monitors factors such as school and business sick-outs, weather patterns and animal behavior. If anything seems out of the ordinarym, it`ll alert health officials.

It`s basically an early warning system so the state can take control of the flu before it gets out of hand.

Governor Dave Heineman says the data the system generates would trigger an alert before medical tests determine there is an outbreak. Three students at the Peter Kiewit Institute in Omaha developed the program. According to a Kiewit official, so far there`s been no cost to develop and implement the program.

Deadly bird flu not forgotten by U.S health officials

WASHINGTON - Less than a year ago, Americans could barely turn on the television, surf the Internet or pick up a newspaper without finding a doomsday story about deadly avian flu.

By last November, President Bush had asked Congress for $7.1 billion to help develop a vaccine, stockpile antiviral medications and fund state preparations for a possible pandemic.

Now, with the disease still centered in Asia and the failure of migratory birds to spread the illness to Europe and North America, the H5N1 virus has dropped out of the media spotlight. The dearth of coverage has prompted some to think that the threat of a pandemic has passed.

Nothing could be further from the truth, however.

So far this year, a person dies from the disease roughly every four days, compared with about once every nine days last year, according to World Health Organization data. Of the 108 confirmed human cases of bird flu thus far this year, 73 have been fatal. That’s up from 97 cases and 42 deaths in all of last year.

“We’re as worried now as we ever have been,” said Michael Osterholm, the director of the Center for Infectious Disease Research & Policy at the University of Minnesota.

Montco Officials Struggle With Flu Mist Program

But an effort to immunize kids in Montgomery County is so far not working out.

All the middle and high school kids in Montgomery County public schools are being offered flu-mist, a nasal spray extremely effective at preventing the flu.

But the offer is falling on deaf ears. At Norristown High School the auditorium is empty.

All of the nearly 2,000 kids could get the flu mist but only three kids showed up.

Mike, his sister Allison and Claire James were the only students who signed up for shots, even though every year 150 mostly healthy kids die from complications of the flu

“In Japan several years ago it was mandated all children be immunized against the flu,” said Dr. Joseph Dimino, a Montgomery County director of health. “And that particular year, there was virtually no flu throughout the entire country. So the adults didn’t get it because the kids didn’t get it. That’s exactly right.”

Wal-Mart to sell $4 generic drugs in 14 more states

Wal-Mart Stores Inc. on Thursday said it would begin selling $4 generic prescriptions in 14 additional states, including New York and Texas, speeding up the roll-out of a plan that has put pressure on rival retailers. ADVERTISEMENT

Wal-Mart said the $4 program covers a 30-day supply of 143 different drug compounds, representing nearly 25 percent of the prescriptions it currently dispenses in pharmacies nationwide.

The program, initially launched in Florida last month, will be available in an additional 1,264 stores throughout Alaska, Arizona, Arkansas, Delaware, Illinois, Indiana, Nevada, New Jersey, New Mexico, New York, North Carolina, Oregon, Texas and Vermont.

Ducks die en mass in Vietnam’s central highland province

Hundreds of ducks in Vietnam’s central highland Lam Dong province have died with unidentified causes, Vietnam News Agency reported Thursday.

Switzerland

Swiss become the first to bulk buy pre-pandemic bird flu vaccine

Switzerland has apparently ordered enough of an experimental bird flu vaccine to inoculate it’s entire population.

In so doing the Swiss have become the first country to bulk buy a supply of GlaxoSmithKline’s bird flu vaccine for humans.

In trials earlier this year the vaccine demonstrated a more than 80 percent strong immune response in test subjects.

The vaccine has an extra ingredient in the vaccine formulation which is designed to increase the immunogenicity of the vaccine as well as potentially offering protection against variant virus strains.

According to Europe’s biggest drug maker the Swiss Federal Office of Public Health has placed an order for 8 million doses of its H5N1 vaccine, enough to protect its entire population in the event of a influenza

Are we ready? A look at emergency preparedness in Saline County/Plan for pandemic response is developed

In June, the Saline County Heath Department took one step in that direction by hiring a part-time pandemic planner, Cindy Meyer. Before that, the department had to depend on an epidemiologist from Independence to provide similar services.

In September, Meyer became a full-time employee as new money became available from the State Emergency Management Agency.

Meyer’s first task was to develop the “Regional Public Health Preparedness and Response Guide,” which she completed in August and submitted to the state.

Experimental Vaccine Protects Mice Against Deadly 1918 Flu Virus

“Understanding why this influenza virus was so deadly is an extremely important question,” says NIAID director Anthony S. Fauci, MD. “By building upon earlier research where scientists successfully reconstructed the 1918 pandemic flu strain, Nabel and his colleagues have demonstrated that this virus is vulnerable to intervention. This knowledge will help further our continued efforts to develop treatments and vaccines to protect us against another deadly flu pandemic.”

Using the genetic sequence information for the 1918 flu virus, Nabel and his VRC colleagues created plasmids — small strands of DNA designed to express specific characteristics — carrying genes for the virus’ hemagglutinin (HA) protein, the surface protein found in all flu viruses that allows the virus to stick to a cell and cause infection. The researchers created two types of plasmids: one to reflect the HA found in the original 1918 flu virus; the other an altered HA protein designed to attenuate (weaken) the virus.

To determine the vaccine’s protective effects, the CDC’s Tumpey intranasally exposed a group of mice to live, reconstructed 1918 virus 14 days after they were immunized with the experimental DNA vaccine. All 10 vaccinated mice survived the challenge with the deadly virus. To explore how the vaccine protected the animals, the researchers first depleted other mice of T-cells; however, this had no effect on the immunity of the vaccinated mice to the 1918 virus. In contrast, the researchers discovered that transferring antibody-rich immunoglobulin (IgG) from immunized mice to non-immunized mice resulted in antibody levels in the animals at levels only slightly lower than those that were immunized. Further, when the animals were exposed to the reconstructed 1918 flu virus, 8 of 10 mice that received antibodies from the immunized mice survived; none of the 10 mice that received IgG from the unvaccinated control group survived.

The study authors indicate that further testing will be needed to determine whether DNA vaccination can confer immune protection in people similar to that seen in the study mice. Additionally, the use of DNA-based vaccines are being explored as a potential strategy for creating vaccines to protect against the H5N1 avian flu virus.

This is the best news I’ve heard in a long time. If an effective H5N1 DNA vaccine can be developed, I will be positively giddy with optimism. It is very easy to produce tons of a DNA vaccine in a very short period of time. You don’t need eggs, you don’t even need cell culture. And even if the virus changes or there are multiple strains, you can develop a new vaccine very quickly. I hope they move forward on more animal challenge experiments with a DNA vaccine to H5N1. Hopefully, they will try it on ferrets.

Monotreme. Revere said today on effect measure that he/she knows of no DNA vaccines currently in use in human vaccines. How safe are these vaccines? I seem to remember that someone mentioned the chance of incorporating retroviruses with DNA vaccines…what do you think?

Write your Congressman and ask for more money for NIH. Full disclosure, my work is funded by NIH, so I’m extremely biased ;-)

Seriously, I do think we need to contact politicians and let them know them know that we think research into vaccines and drugs that can blunt a pandemic is important to us.

I think that’s true. However, there are DNA vaccines that are being used successfully in animals. Also, I believe there are experimental human DNA vaccines that would be used, if necessary.

How safe are these vaccines?

Hard to say, given no data in humans. In general, they are probably relatively safe. However, I would think that there is a possibility that the construct DNA could end up in a germline cell, although this is probably pretty unlikely. I would also think there is a possibility that it could integrate into nuclear DNA in something like a tumor suppressor which might cause cancer. However, this is also pretty unlikely. However, to be on the safe side, I think old farts like us should volunteer to be injected first. I also think that if the pandemic is severe, I’d take death from cancer in 10 years over death from flu now.

I don’t think so, unless someone plans to use retroviruses as the vector to the DNA in. My understanding is that this is not necessary since a transient transfection is all that is needed. I think the plasmid is blasted into skin cells with a high pressure device or simply injected with a tuberculin needle. So, retroviruses are not needed.
