Man you people are great!! I am really having trouble following this, much less understanding the translation. One thing though, is that 1 month old baby. I had no doubt that he did not come into contact with a bird, ex. play with, did not eat any bird. Baby was no doubt nursing. Let me just pose this question. If the parents are infected, could the mother pass it along in her breastmilk? O.K., a little gross for some guys, but nursing passes on alot from whatever the mother ingested. If she ate infected chicken, would something show up in her milk? Unless, (parents, family members), sneezed on the baby. I can’t find one logical conclusion for that baby to have gotten infected, if he/she is.

MALANG, East Java: Ashfalls from Mount Semeru in East Java have had a severe impact on air quality in the region, resulting in more than 1,000 cases of pneumonia in the last 10 months.

The head of environmental health and disease prevention at the Malang, East Java, health office, Mulyatim Koeswo, said Tuesday that 75 percent of the pneumonia suffers were children, while the rest were adults and teenagers.

He said pneumonia was triggered by untreated respiratory illnesses. In certain cases, the condition could lead to death, especially in children under the age of five, he added.

Mulyatim said the children affected by the disease were mainly those who had not been immunized against diphtheria, whooping cough and tetanus. He said the disease easily affected malnourished children.

“Currently, there have been no deaths from the disease. All (cases) have been handled by the medical team at the hospital or community health centers,” Mulyatim said.

Pneumonia’s symptoms include a rising temperature, up to 38 degrees Celsius, breathing problems and a worsening cough. According to Malang health office data, between January and October this year 1,176 people in the regency have suffered light to serious pneumonia.

Mulyatim urged parents to immediately seek treatment if their children showed such symptoms.

He said it was important that people maintained clean and healthy lifestyles and not smoke around children. “For infants, breast milk will help them avoid getting the disease,” he added.

He also asked residents wear masks while outside their homes, especially when riding motorcycles.

“Mount Semeru’s volcanic ash can also cause pneumonia,” he said.

Today is 10/20. There seems to be two trouble spots. South Sulawesi and North Sumatra. Let’s go back 10 days to 10/10. We have had 4 patients in South Sulawesi. Of these 4, 2 have been under 6 months old. In North Sumatra, we have had 5 patients in the last 10 days. Out of the 5 patients, 3 of them have been 3 years old and under. So between South Sulawesi and North Sumatra, we have had 5 children under the age of 3 years old in the past 10 days.

The article posted by Commonground at 6:06 references a confirmed positive death from the villager the Urip Time. I did a little digging and found that this statement is in reference to Mistinem. This is an exerpt from the pugmom post at 16:01 on the Outbreak XV thread:

“Purwokerto – Mistinem (27), the citizen of the Giana Mountain Village, the Urip Time Village, the Madukara Subdistrict, the Banjarnegara Regency, on Friday (13/10) at dawn struck 03. 00 WIB died it was suspected resulting from bird flu.”

The new patient (40M) is stated as being from Triguno and is now at the hospital in Semarang. I couldn’t locate Triguno, but Semarang is a little over 60 miles (as a crow flies) from Sigedang, the location listed for Mistinem. Can anyone identify where Triguno is? I find the reference to Tamiflu in the community at Commonground’s 6:06 post a little odd.

I have been looking with diligence for maps which can tell us where Triguno is, and where the small villages cited in Goju’s report are located as well. All these villages seem to be too small to appear on any maps so I have had no luck yet.

However, during my search for these villages I did come across a very interesting FAO Document: Geospatial Analysis of HPAI (H5N1) Outbreaks in Indonesia from 2003 - 2005, that was published Feb. 2006. http://tinyurl.com/y56776

I just want to point out that the new spokesperson we are now hearing from, Runizar Ruesin, does not seem to be lending clarity to the situation. His pronouncements are often a bit “off” and while are mostly accurate, are not wholey accurate. Fuzzy pronouncements coming directly from the head of the Health Ministry’s Avian Flu Center is the last thing we need and the first thing that will engender lack of confidence in, and nervousness about, how Indonesia is handling things. That’s an excellent question and maybe an answer to one of my questions. The thing that jumped out at me after one of the stories about a baby becoming infected (and I wondered aloud how that could have happened) was that they said in a followup story that the family’s chickens had been kept in the cellar. I wondered at the time why that would be the case as it seemed so strange, but maybe your theory about the rainy season is right.

I still don’t think a two-month-old baby would have had much contact with chickens, however, and there are no report that that family’s chickens were ill or died. The only reports we have seen so far are that the baby died, the parents were being tested, and the mother’s initial test results showed she was not quite as healthy as the father, but they put that down to the Ramadan fast (although I’m not quite sure nursing mothers are either expected or required to participate in the Ramadan fast as people who are ill or have special health needs are exempted).

One thing that I do know is that if I ever get into trouble, I think I want an Indonesian defense attorney. They seem to be very creative in thinking about ways to direct attention away from not thinking about the obvious.

I was reading earlier today on RSOE, that they are expecting cooler weather in India and that they thought it would help the Dengue problem. I am posting it below as I think it also applies to Indo. I think I will start checking the weather in various Indo areas to see what the temperatures are. Once things “cool off” the mosquitoes should start to die and there should be an immediate drop in Dengue and Chickungunya cases. Once the weather cools, if the numbers of the cases continue or increase, then it may very well be something else. JMHO

I have went back and tried to catch up on all today’s news. I don’t see in either article about the 40 year old manwhere he travelled to for Ramadan. Is that just an oversight by the reporters or an intentional omission? What I would really like to know is where he went, how he got there and how he got back. I thought I saw an article about that on Bloomberg before I left for work but I was running late due to posting that joke article about backyard chickens in Indonesian cities. Now about the babies I really cannot guess, I don’t know that the officials would tell the truth about anyone else in their families being sick unless they absolutely had to. All this talk of babies getting sick. Look this is not suburban USA. Babies are not being placed on clean blankets in the grass or scrubbed linoleum floors. These folks are living in rural villages where chickens are in and out of the house. They probably have dirt floors, or at best wooden floors. Chickens poop everywhere. (Believe me, I have chickens and they poop about every 10 minutes!) Kids are rolling around on the floor. This is not a sterile situation. I would actually think the babies would be the first to get in contact with germs from the chickens. Adults probably are a bit more careful to clean their hands before sucking their thumbs! ;)

Results of the serology test from the Hall of Health Research And Development (Balitbangkes) against the patient suspek bird flu, Dn (3 years 8 months), that was treated by RSUAM, pointed out results of the negative.

However, the team of the handling of the RSUAM bird flu patient was still being waiting for results of the PCR test (polimerase chain reaction) from Balitbangkes because, pneumonia on the Dn lungs continued to spread.

Sounds like the kids are only coughing, but staying at home. Also sounds like the only test we will get back this afternoon is the blood leukocyte count. (in viruses, the leukocyte (white blood cells) is low. in bacteria, the leukocyte count is high.) IE—this is a very non-specific test for viral infections, and certainly not a specific test for H5N1. (so don’t hold your breath.)

So maybe they are at home, since the parent doesn’t seem to believe it could be AI maybe she did not permit them to be hospitalized. Who knows anymore? It was interesting to note that in South Sulawesi, both Fajrin (2 months) and Asrianti (3 months), were both from Maros Regency. I have to see if I can find out what Village Asrianti is from. Fajrin is from Bontopaddingin/Bontotallasa.

Fatal Suspect H5N1 Cluster in South Sulawesi

The above translation describes a suspect fatal H5N1 bird flu case in South Sulawesi. In addition, at least two family members are hospitalized and several contacts are being tested. The H5N1 situation in South Sulawesi remains unclear.

Last month a fatal confirmed case from Mekassar was announced. However, the patient had died in June. The WHO update indicated the delay in reporting the confirmation was because it was collected during routine testing. However, the WHO update failed to mention the deaths of two family members who also had bird flu symptoms. Thus, the failure to report, couple with the misleading characterization by WHO is cause for concern.

Between the earlier fatal cluster and the current suspected fatal cluster, a large number of suspect cases have been hospitalized in South Sulawesi. Although these cases have tested negative, patients have been treated with Tamiflu which may lower the level of circulating H5N1 to a level that is below detection. False negatives are common in H5N1 cases and frequently multiple tests are required to detect H5N1. A recent confirmed H5N1 fatality in Thailand was tested nine times over several weeks, yet H5N1 was only detected at autopsy.

The frequency of H5N1 infections in Indonesian remains unclear. The H5N1 in patients does not match the H5N1 in poultry. Since a link to dead or dying poultry is usually required fro H5N1 testing, the level of H5N1 infections in patients with bird flu symptoms, but without a bird link is largely unknown.

If these patients have been infected with H5N1, they will have antibodies that can be detected 3–4 weeks after disease onset. It remains unclear how many of any surviving patients with bird flu symptoms are tested for convalescent H5N1 antibodies.

Similarly, sequence data from H5N1 positive cats has not been released. The only cat H5N1 sequence released to date does match the human H5N1 sequences, which provides additional evidence for a mammalian reservoir. Similarly, analysis of recently released H5N1 sequences in China show that recombination is frequent, and regions of identity are found in Indonesian patients, providing evidence for the introduction of new polymorphisms in Indonesia via migratory birds. H5N1 sequences from wild birds in Indonesia are also lacking.

The latest suspect cluster in Indonesia again highlights the need for expanded surveillance and more timely reporting.

Recent media reports indicate that there will be testing of waterfowl in the Trisik area, Yogyakarta, and the Eretan Coast, Indramayu. Thus far the only reported bird H5N1 from Java with the novel cleave site was from Indramayu. Although it did not match the majority of human cases, it was close to a small subset of patients infected at the end of 2005. The only other bird isolates with the novel cleavage site were from central Sumatra.

 Although the novel cleavage site has not been detected outside of Indonesia, many polymorphism from patients in Indonesia have been detected in H5N1 in China, strongly implicating these isolates in the evolution of H5N1 worldwide, including several polymorphism that were widely detected in Indonesian patients.  Moreover, the origins of these H5N1 sequences in China could be found in low path isolates in Hong Kong in the late 1970’s indicating low path flu sequences are also closely involved in the evolution of H5N1, which is largely driven by recombination, which was readily detected in the H5N1 from poultry, waterfowl, and swine in China.

Just my 2 cents but I think there is the 40 year old ex-pat, 12 year old Alfian, a death at the temple village in this month and a death at Blaru village in September just now reported and the last two were thought to be dengue but now maybe AI since Blaru villager was thought to have suffered same fate as Temple Villager who at the time was thought to have dengue. Just my two cents also but I’ll bet they all had contact with sick chickens too. Notice too that they mention the rain in association with the dengue but as we all have learned rain is also associated with a rise in the AI cases.

Don’t forget Mistinem the 27 yo woman who died and was from Kaliurip. (Thanx Milo for the time explanation.) But I guess I am nervous as we have 2 people from Kaliurip that are dead. It is either 1. Mistinem and an 11 yo boy whom we don’t have a name on OR 2. Mistinem and Alfian (and they got his village wrong.) Other than that, I agree with your assessment at 2008 Influentia2. However, let’s not forget 21 yo Parmiun admitted Oct 13th and was from the village Krinjing, and was the one ill and confused and short of breath for months before being hospitalized. (I still think he might be an asymptomatic carrier.) I remember him, as I spent the better part of half a day trying to determine if his village of Krinjing was close to Mistinem’s village of Kaliurip (as I recall they are about 30 miles apart.) But at the time everyone on FW was wondering if there was a link between those 2 people. Now we have another death from that same village of Kaliurip. I guess we need to research to see if the village Kaliurip is close to Triguno or Sumberejo.

There are numerous “populated places” with the name Sumberejo around Yogyakarta and between it and Semerang. In short, Kaliurip is within 60 miles or less of the places labeled Sumberejo.

I looked the article that mentions the 40 yr male in conjunction with the word Triguno. It appears that he had just returned to his families home for Ramadan a few days before he got sick. He very well may have caught the virus somewhere else before he traveled back home for Ramadan.

Here is my dilemma: I read this as the death of the 11 year old boy/girl was in the same notation as the Urip Village. In other words, I thought the 2 phrases were related. I have surfed all over CE and I now feel there is no connection to these phrases. I think the 11 yo is Aulia from Jakarta. If so, that means there has only been one death from Kaliurip and that the 11yo (presumed Aulia) is different than Alfian. Can anyone here confirm that? There are two situations independent of the other going on here. Anyway that is my two cents about this. Trying to link the two areas/people together I don’t think will work unless they all were on one of those diahrrea trains together going to Ramadan and I am just kidding about that really.

It unfortunately sounds like these children were too poor to even be “run off with” to a community hospital, (nevermind an H5N1 reference hospital), even though the usual question about whether birds had died nearby was answered in the affirmative. That these siblings both became mortally ill at almost the same time, and died at almost the same time, makes an outside agent other than just malnutrition suspect here. It’s really hard to say what is going on - even untreated otis media, or a simple ear infection (they mention ear inflamation), can progress if untreated to fever, vomiting, and even meningitis-like symtoms (although most children are treated far before that point with antibiotics and they may have not been here). But still, what are the chances that two children, reported healthy beforehand, would die of something like that within two hours of each other?. What outside agent could act upon them both so quickly? Does the sick poultry nearby implicate H5N1? Most likely we will probably never get a clear diagnosis in this case since there has been no involvement, and thus no implied responsibility, of a doctor and medical clinic or hospital. If not for the chance mention of the chickens, we would probably have never even heard about these children’s deaths.

It is really odd, and frightening, that the mother said the children were both well and were playing just hours before their deaths.

Terrible—those 2 little boys died within 2 hours of each other. I don’t know what cephalic tetanus is (so preventable with routine vaccinations). I will go research that particular definition of tetanus. Tetanus usually doesn’t kill that fast—usually a long, draw-out, painful death. The only thing I know that kills children this fast and with this much rage is meningococcal meningitis/septicemia. They were symptomatic one day, dead the next. Meningococcemia kills that fast. It is also contagious. Awful, awful they were not rushed to an intensive care hospital.

I am not sure what kind of spray they are using in Indonesia, but here people who raise domestic animals use things like Virkon-S as a disinfectant spray and my understanding is that it is effective against many viruses. China also routinely disinfects the areas in which there have been H5N1 outbreaks with disinfectant sprays, and plenty of it from what I can gather.

I assume routinely cleaning and disinfecting a chicken coop would be a good idea if one feared H5N1 infection. If wild birds bring HPAI H5N1 to North America, that will probably be exactly what farmers and backyard poultry owners will do even more frequently than they do now. However, in Indonesia such disinfecting is expensive, and is probably beyond the means of almost everyone. I have not heard of rigorous spraying, as they do in China, even when H5N1 infections are found in flocks (they may do it, but they do not stress that they do it if they do, while China typically releases photos of many men in full PPE spraying everything in sight). Why they are releasing photos of the recent spraying in East Java I don’t know, but I do know that the President of Indonesia is visiting farms in East Java during his holiday “safari” there this week (along with the Minister of Agriculture). Not that I think they’d do anything special for them or anything.

The location of the 2 children is said to be Pemalang. That is in Central Java. As a crow flies, it is about 10 miles from Sigedang. I show Sigedang to be where Mistinem (27F) lived. Mistenem showed symptoms of pneumonia on 10/08 and died 5 days later. She tested positive for H5N1.

Pixie at 22:37… I haven’t listed any of the locations as Pati. Perhaps I’ve captured a different level of data (i.e. town vs regency vs ?… I can hardly keep it all straight.)

On the Pati connection, North Central Java, in a 30 mile wide triangle on that peninsula we have:

1. Mystery Man (40) Triguno Village, who had been visiting in Puncakwangi Subdistrict, Pati. Treated first at RS RAA Suwondo Hospital, Pati 17/10, then run off with to Karidari Hospital, Semarang, when his condition worsened 18/10. Respiratory tract/ isolation. Several chickens died suddenly on 12/10. Has tested negative.

2. Alfian (12) from Sumberejo village, Rivet subdistrict, Jepara regency (Influentia2 - 17:02). Treated first in RS Kelet, then due to poor condition immediately transfered to RS RAA Suwondo Hospital, Pati. Brought there on 10/10 and deceased 10/20. (Jepara is about 20 miles from Pati).

3. A dengue (DBD) fatality from Temple Village, Pati subdistrict.

4. Another dengue (DBD) fatality in Blaru village in Sept.

We also had the preschooler, Bimo Alamsyah (2.7) from Kedungpane districts, the Mijen Subdistrict, Semarang (about 25 miles from both Jepara & Pati). He was treated in isolation space of RS Kariadi, Semarang on 30/9, given Tamiflu, then was stated negative (3X) This was the case where the family’s poultry died, but also 6 chickens, 7 goats and 4 cats died suddenly and they ate the goat, including Bimo.

Halo Commonground- That is good new. Maybe we will get lucky and this will die out. India is reporting things are settling down too. Let’s keep our fingers crossed.

Take a deep breath and enjoy the lull. I’m afraid when all the city folk have returned from their holiday in the country we will see a flurry of new cases. I figure the end of this week it will begin picking up in a big way (not necessarily pandemic). Hopefully I am wrong.
