I stumbled onto this survey out of Canada by following a link here somewhere that led to another link about nanomasks and then another link, and eventually got to a Google search list where I found this link. I took the survey and thought others here might be interested in either taking it or following up with the group to find out ongoing results, etc.
It was a little confusing to answer some of the questions because I’m here in the USA and we don’t have a problem with a direct BF threat right now on our soil, and the questions are geared for anyone in the world to answer, so anyway, just to forewarn you that it’s going to be a little hard to answer some of the questions.
I put the info that was on the ‘survey complete’ page at the bottom of this post. They offer the CDC and the WHO links for further info — maybe they’d link to us as well since CDC spoke of us here.

I did that survey about 6 months ago when I first saw it but I have not seen any ‘results’ from it - has anyone?

I took it but found it confusing as right now I am not worried about catching avian flu nor would I be freaked if some chickens in the US came down with high path. A pandemic would be a whole nother ball of wax. Then I would be in freak out mode in answering some of the questions.
I had that reaction too, but I did the best I could — figuring that since it asks what part of the world you live in, they would account for our ‘lack’ of panic because we’d not been exposed and had no reports in our country. I found the questions fascinating.

Yeh…count me as a participant. Good you posted it because if it makes a change then I’m glad to be a part of it. Get’s you down answering the same question 5 times, but I suppose they have some kind of method to their madness. I said that I didn’t know the symptoms…also said that mainstream media has delivered a fair bit of information. Said that forums like this gave you more because it can be collated in these types of things.] Good survey really. I totally recommend it to anyone. :)
