I didn’t find a lat and long for another Dua Boccoe, but Wikipedia lists a Dua Boccoe in the northern part of Bone, and I found a plain vanilla Boccoe in the Bone Regency. It seems to be maybe 30 km west of Watampone. Latitude: −4.4569444 Longitude: 120.2352778

I just found a great site for an emag called archipelaGo (http://tinyurl.com/o7twv). On the main page, one of the choices is Provinces (http://tinyurl.com/s76vj). You can then pick the province, go to the tourism page, and find lots of place names and relative locations. It said Boccoe is 34 km from Watampone and is famous for Mampu Cave, the biggest cave in South Sulawesi, and it’s inhabited by bats. Hmmmm…

If anyone is able to find a site that shows the Regency and Sub-Regency (District and Sub-District) boundaries of Indonesia it would be very helpful to me. I have the Provice data in my GIS map project for Indonesia. I have not been able to locate Regency and Sub-Regency data (shapefiles or geodatabase) to incorporate into the project. Short of that, a site that shows these within each province (without having to drill down for info time and again) would be helpful. Thanks.

Metrotvnews.com, Bandung: the Handsome Sadikin Hospital (RSHS), Bandung, West Java, again treated one patient suspect bird flu.

The patient was named Muhammad Rifki Farhan from the Cipasung Village, the Kuningan Regency, West Java. The patient is currently treated in Poinciana Tree space that was special isolation space the bird flu sufferer. By making use of the ambulance, Rifki arrived in RS Handsome Sadikin, on Monday (4/9) around struck 16. 30 WIB. Rifki could be beforehand treated in the Kuningan regional Public Hospital. Here is more concerning the suspect case in Cipasung village. Please note that the patients name, Raihan, is translated “Brass” in the article. The speed of on-set of symptoms following the death of the chickens is of some interest I would think. Also please note the even though the title mentions “pre-schoolers” I believe it is a matter of translation only and is not actually in the plural. There is only one suspect case mentioned.

I was thinking this was the same case as posted at 12:16. I am not so sure after reading them again. This might would also explain the plural term “pre-schoolers” too. This bears watching closely in my opinion.

I’m finally on the ‘puter, so I’ll work on a lucid summary of South Sulawesi.

I can answer the questions about geography now — I think the east-west confusion is because South Sulawesi is the west leg of the island, so it has both a west coast and an east coast, but the east coast is not the east coast of the entire island. (Who’s on first?) Tanete and Boccoe (aka Dua Boccoe or Two Boccoe) are towns in the Bone Regency, both close to the east coast. Tanete is about 70 km southeast of Maros (which is on the west coast), and Boccoe is about the same distance northeast of Maros, so they make a triangle. For Boccoe, look 34 km west of Watampone.

On the map, the brown lines are roads; larger roads are solid, smaller roads are dotted. If you click on “Legend,” it’ll show you what all the symbols are. The political divisions are gold strips with black lines or dots. If you zoom out and go north a bit, you can see what they look like at the boundary between South and Central Sulawesi.

The negative patients are part of the Palu group, and I’ll see if I can find more details.

I think maybe it’s the calm before the storm literally. There are several articles referencing the impending monsoon season. Officials seem very concerned about the rain spreading the virus. I will be keeping my eye on that and also these stories about people throwing infected chicken carcasses in the rivers and these cures that are popping up. I’m trying to look at the big picture here, even if it isn’t obvious just exactly what it is yet.

I figure that if I am going to search for “cases” I may as well check out things that interest me or maybe someone else. You never know what piece of information may end up tying things together somewhere.

Otherwise, news was slow, I had time to make 24 quarts of homemade vegetable soup to put up. YUM!!!

These people throwing chickens into the rivers bothers me. How many chickens thrown into the rivers are too many? Or maybe it’s nothing. If the Indonesian farmer thought of it, maybe the Thai, Cambodian, Vietnamese farmer etc. did too. I have read the virus can survive in water up to 35 days.

This upcoming monsoon season bothers me too. You can sense the concern from the Indonesian officials in the articles when you read them.

I think you just never know about these things, anything is possible at this point. Plus this is just Indonesia, no one knows much if anything about Africa or China. Sometimes it is like we all have our wagons in a circle, you don’t know which side you will be attacked on, or more than one side!

I will be watching this situation as closely as I can. Good night, back to work early tomorrow.

On the Bone puzzle, I’m no mathematician, but I don’t see any way to make the new numbers add up. It says that between August 24th and 28th they sent 4 blood samples of people who owned positive chickens, then 7 more from people who had had direct contact with positive chickens. That makes 11. Then there’s a list of those whose samples were sent, and there are 9 people listed (5 from Tanete and 4 from Boccoe). An official said he got some unspecified number of negative test results, which he expected because the people weren’t showing symptoms, then he said all the blood samples were still being checked.

The only possible explanation I can think of is that they sent two samples from unnamed people on the 24th, which the official received negative results on. The 9 named people had samples sent later, and they haven’t received results yet.

If I got it right, quails harbour two types of sialic receptors (alpha-3 and alpha-6) in the airways. Humans have got one kind in the lower respiratory tract and the other in the upper respiratory tract. At the moment, it seems that the virus is only well adapted to the receptors located in the lower respiratory tract, which by beeing deeper makes it harder to transmit to other humans. It is thought that bird flu would be more easily transmitted if the virus mutates to being able to infect the upper respiratory tract receptors.

The article you have quoted is therefore very important. It may mean that quails are not only more susceptible, and therefore more contagious, but they are making it easier for the virus to mutate to a capacity to infect the receptors located in the upper respiratory tract.

Snowhound1, I used to believe no news is good news but considering the fact that H5N1 in it’s numerous incarnations is endemic throughout Asia(my personal mind picture of the situation is of a combination lock, with Nature spinning the dial),it’s hard to believe nothing’s going on. I think it far more likely that it’s just not possible to be aware of every case, every situation- the best we can do is try.

I can’t get over the amazing job that’s been done keeping up with all this, it has to be extremely difficult and confusing to say the least.

We had 5 patients listed from Palu, so this is good news. And they are declaring that it is still in stage II, ie still in poutry and not moving out into “humankind.” However, the next paragraphs are mysterious and disturbing. Undata is (after googling it) the name of the hospital near Palu, near Palendu, in Central Sulawesi. It may be the same hospital system from which the 5 negative reports were issued. However, in this paragraph, it seems they are talking about extreme reticence on the part of the Hospital staff to talk about the situation. They intimate there is a problem with communicating the true status of what is going on, even with the regional authorities. It may be just the usual turf wars, it may be more serious (ie, they know something and don’t want it to get out.) How do the rest of you folks read this??

An article you posted yesterday at 17:42 says Muhammad Rifki Farhan improved and one at 10:36 states that R improved. The article you posted today says that R, Brass was was still in ICU on Wednesday (6/9).

This would make me think there were two patients, except an article posted by Okieman at 17:42 (near the top of this thread) says “Raihan was the only patient “susfect” bird flu that just the first time this happened in the Kuningan Regency.” There are articles on both names saying they started out in RSUD Kuningan amd moved to (RSHS) Bandung.

Because of this quoted statement, I’m still listing only one patient, but will show in ICU.

On Monday, a two-and-a-half-year-old toddler suspected of having bird flu was rushed to Hasan Sadikin General Hospital in Bandung. The toddler’s family lives in Cipasung.

The deputy head of the hospital’s bird flu prevention unit, Djatnika Setiabudi, said the toddler was improving, with his temperature returning almost to normal. However, he is still showing symptoms of pneumonia.

We heard about this a few days ago (over a week now). WHO is accepting verified results from the local labs in Indo and does/will include them in their cumulative numbers. Last I saw, they were sending them to 2 labs in Jakarta and the NAMRU lab. They announced that they were no longer goign to be sendign to Hong Kong necessarily, and that the option could be NAMRU and/or CDC Atlanta now for outside confirmation. But the news report made it clear that the WHO was “believing” jsut the 2 Indo verifications and wasn’t requiring the outside testing to prove the accuracy any more. They stated that those 2 Indo labs they are usign have had 100% accuracy of test results thus far, so I guess they proved they could adequately handle proper testing.

OKbirdwatcher-this Reuter article tries to answer your questions . they claim that the blood sample of of the girl was one of the blood samples “taking during routine surveillance of people with mild influenza symptoms. later in the article “when we found one of them was positive, we traced it and found the girl had died”. so it sounds to me that because those were samples of folks who had mild symptoms, they did not give it priority. worrywart, it’s possible that the symptoms that she presented with before she died were not typical of H5N1. For instance if she had severe diarrhea the cause of death might have been assumed to be something else, perhaps cholera, not influenza. It would be interesting to find out what her symptoms were before she died and what they thought killed her.

Ocean2--they are, of course, not including people that had the cardinal signs of BF, were not tested, but died anyway. Just on our last summary from last week that Michelle has complied, there are 5 patients that had no tests, yet died with the cardinal symptoms. (Just since late July.) Since late July we have had over 81 presumptive cases of which only 4 have been declared WHO positive and have died. That leaves many of whom we have heard nothing back on, many (15) that have tested negative and survived, but may have been survivors and negative testers because of the Tamiflu blanket. I haven’t seen the latest update summary that was put out this am—I must go get it—but I bet the figures are even higher. I can’t help but thinking as we run all these numbers, so many conflicting and confusing, that it is the “camel’s nose under the tent” syndrome. If you haven’t seen the spreadsheet work Michelle is doing, by all means, go take a look, it is on the wiki. The map is pretty startling too and is at the top of the page on outbreak summary thread. What do you personally think??

Ocean2--more clarification—the 5 patients that died without testing met one of the WHO criteria for suspect cases. That is: they had exposure to infected poultry AND they had pneumonia. With 29 out of 33 Indonesian provinces now having infected poultry, it is not too hard to meet the WHO exposure criteria. Apparently the WHO is now accepting confirmation if it done by 2 separate Indo labs. My understanding is that a fairly quick PCR test can be done within 3–5 days, but positive confirmation must then be further done by growing out the virus in culture or serial antibody testing done starting at 2–3 weeks—each of which takes time, and must be done by those two specialty laboratories. Also, Tamiflu cna interrupt all these testing procedures.

and there are other articles telling of severe water shortages with people having to use dwindling and at times unsafe sources of water for drinking, crop failures(rice, mung beans and corn), loss of stock and spaces to breed fish and prawns, poor or no pasture for cattle …..in other words communication, sanitation, food and clean drinking water are all in seriously short supply in many areas right now. I feel for the people of Indonesia, what a terrible hand they’ve been dealt.

I thought Reny Boru Ginting 33 might have been Rasmita Boru Lingga, but there is no way, plus the mother wasn’t pregnant. The first few times I saw the map I really just cried. Not because of what it predicted for us here in the West, but for all the people who live in those places. What they must be going through……..Here in the Netherlands live many people of Indonesian descent; they are beautiful and graceful people.

I can imagine that it may affect you deeply, as you work so closely with the news reports-it would be hard to view it dispassionately.

I think the three people are really just one — Akira (also spelled Akhira in one place), a 14-year-old girl who one source referred to as a woman. Except for the one “boy” report, and a date discrepancy (one source said she got tested 6/24/06, and another said she died 6/24/06), all the details are the same, so I think it’s just the one person.

We knew about those cases from Sumatra, we were lurkers then. I sent you the articles, I still have them. Some of these are the kids that were taken by their parents and then sued because they were diagnosed with BF which made them outcasts. Some were neighbors. Remember there even was a picture of an empty hospital bed?

Anyway, if you think what is going on now is weird, just wait until the monsoon hits. There is probably a lot we don’t know and never will going on over there, but we can make some pretty good guesses.

When I saw the new death my first thought was that they are probably just releasing news slowly to desensitize(sp) the masses. After the average citizen hears a few more deaths he will be used to it and less likely to panic. Just my opinion though because I think that is how a lot of news works.

COMMENT: I’ve been looking over the cases to fill in gaps in our understanding, and the patients Hendra (21) and Gilang (9) have always concerned me. We have no information on them except that they are from Garut. We aren’t even sure of their gender. They are the only patients which have tested positive but have not yet been listed on the WHO website.

The thing that bothers me is this. Cikelet is in Garut. If you have cases testing positive 1 1/2 weeks after the Tamiflu blanket, that is of concern.

I searched flutrackers.com and found an article posted by AlaskaDenise. Link to article Here are some excerpts:

“In Garut, the virus first appeared in the villages of Cikelet and Cigadog, but it has since spread to other villages. The number of suspected bird flu victims continues to rise each day…

Hendra, 21, and Gilang, 9, were the latest victims to test positive for the bird flu virus. They received medical treatment at Dr. Slamet Hospital in Garut. Four others are under observation in their homes by Health Ministry teams. If their conditions worsen, the teams will bring them to the hospital.”

We have 4 others from Cikelet who were hospitalized after 8/25/06, the date of this article. I think it is possible that Hendra and Gilang are also from Cikelet.

Specimens should be collected from suspected cases before or soon after medicines such as oseltamivir are administered, said de Jong, head of virology at an Oxford University clinical research unit in Ho Chi Minh City. Nasal and throat swabs from Vietnamese patients who responded well to oseltamivir, marketed by Roche Holding AG as Tamiflu, were unable to detect the virus 48-to-72 hours after beginning treatment, he said. `If a patient is on oseltamivir for three days before the first swab is taken for diagnostic testing, it’s possible the result will be negative, but the patient could be infected’‘ with the H5N1 strain of avian influenza, de Jong said in a telephone interview yesterday. A delayed or incorrect diagnosis is a concern for disease trackers as they monitor human cases for signs H5N1 is becoming contagious to people, not just birds. A mutated virus spreading easily from human to human might spark a pandemic that could kill millions of people. Since 2003, H5N1 is known to have infected 240 people in 10 countries, killing 141 of them, the World Health Organization said yesterday. Almost all human H5N1 cases have been linked to close contact with sick or dead birds, such as children playing with them or adults butchering them or plucking feathers, according to the Geneva-based agency. Early Detection Early detection and isolation of suspected cases are important to limit the spread of the virus should it begin to spread easily between people, the WHO said last year in a recommendation of strategic actions to fight a pandemic. The most recent H5N1 cases occurred this month in a remote village in the Garut district of Indonesia’s West Java province. Indonesia’s Ministry of Health confirmed H5N1 in three villagers, two of whom died, prompting officials to investigate whether the virus had spread directly between the people. No evidence of human-to-human transmission has been found, Health Minister Siti Fadilah Supari told reporters in the capital, Jakarta, today. Authorities distributed Tamiflu to about 2,100 villagers in Garut, the Jakarta Post reported today, citing Fatimah Resmiati, a spokeswoman from the ministry’s West Java environmental health office. Indonesia, the world’s fourth-most-populous country, accounts for a third of the 141 known H5N1 fatalities since 2003.
